载脂蛋白A-Ⅳ在代谢综合征中的应用前景

摘　　要：代谢综合征的日益增多,已成为一重大的公共健康问题和社会问题。代谢综合征是一组以肥胖、高血糖、血脂异常以及高血压等聚集发病的临床征候群。大量实验研究证明,代谢性炎症和其导致的胰岛素抵抗既是过度营养所致肥胖的结果,又是代谢综合征发生发展的重要原因。研究发现小肠分泌的载脂蛋白A–IV(Apo A-IV)具有抑制食欲、抗炎、抗氧化、促脂代谢及降血糖作用,已成为代谢综合征研究新热点而受到关注。最新实验结果显示,Apo A-IV降血糖的作用部分是通过刺激胰岛素释放和抑制肝糖合成实现的。笔者发现,Apo A-IV可增加胰岛素敏感性,但抑制炎症信号传导通路。因此,预测Apo A-IV可能在代谢症候群的发生和发展过程中起抑制和保护作用。本文将根据代谢综合征的发病机制,结合以往研究报道和作者的最新实验结果对Apo A-IV的作用进行评述,并预测其在代谢综合征中的应用前景。The increasing global burden of obesity and its associated metabolic syndrome have become one of major public health problems and social problems. The metabolic syndrome（ MS） is a cluster of obesity associated metabolic derangements such as abdominal obesity,hyperinsulinemia,impaired glucose tolerance,gluconeogenesis. The author‘s preliminary data showed that Apo A- IV can improve insulin sensitivity and inhibit proinflammatory pathway as well. Thus,the author predicts that Apo A- IV might play an inhibitory and protective role in the generation and development of the metabolic syndrome. According to the pathogenesis of the metabolic syndrome,combined with the previous reports and author＇s latest study results,this paper will review the function of apo A- IV and predict its application prospect in the metabolic syndrome.
