高血压并左心室射血分数保留型心力衰竭患者全心功能指数与B型脑钠肽的相关性研究

摘　　要：目的探讨高血压合并左室射血分数保留型心力衰竭患者全心功能指数（Tei指数）与B型钠尿肽（BNP）之间的相关性，寻找高血压患者早期心功能受损客观诊断指标。方法将98例高血压并左心室射血分数正常的患者分为舒张功能正常组及舒张功能减退组，两组分别抽取晨间空腹静脉血检测BNP水平。根据心脏超声所二尖瓣口血流频谱舒张早期血流（E波）峰值／舒张晚期血流（A波）峰值、二尖瓣口血流频谱舒张早期血流（E波）峰值／二尖瓣环组织多普勒舒张早期（E’波）峰值、肺静脉口血流频谱收缩期流速峰值（s）／舒张期流速峰值（D）和Tei指数，再将舒张功能减退组分为轻、中、重3个亚组，比较各组BNP及Tei指数差异，分析BNP与Tei指数的相关性。结果高血压合并左室射血分数保留型心力衰竭患者BNP值、Tei指数随舒张功能减退程度加重逐渐升高，各组BNP与Tei指数之间呈明显正相关（r=0．786，P〈0．01）。结论Tei指数结合BNP值可作为一项临床评估高血压患者早期心功能受损方便、快捷、有临床价值的指标。Objective To evaluate the correlation between the whole hearted functional indexes （Tei index） and the B-type natriuretic peptide （BNP） among hypertensive heart failure patients with preserved left ventricular ejection fraction, and to survey the objective diagnostic indexes of the loss of heart function among hypertensives in the early stage. Methods 98 hypertensive patients with normal left ventricular ejection fraction were divided into 2 groups, one was patients with normal diastolic function; the other was patients with decreased diastolic function. Sample the venous blood from two groups with an empty stomach in the morning to test the BNP and examine the index of E/A, E/E, and Tei by cardiac ultrasound. The decreased diastolic function group were divided into 3 subgroups：light, medium, heavy. Compare the Tei index and the BNP, examine the statistics differences. Analyze the correlation between the Tei index and the BNP. Results With the diastolic function decreasing, the BNP and the Tei index among the hypertensive heart failure patients with preserved left ventricular ejection fraction increase gradually. The BNP positively correlated with the Tei indexes in each group （r = 0. 786, P 〈 0.01 ）. Conclusion The Tei index along with the BNP can be regarded as a convenient, fast index of clinical evaluation to evaluate the loss of heart function of hypertensive patients in the early stage.
