活血潜阳方对高血压血瘀阳亢痰浊型患者胰岛素抵抗及颈动脉斑块的干预作用

摘　　要：目的:观察活血潜阳方治疗高血压、改善胰岛素抵抗及颈动脉粥样硬化的作用。方法:选择高血压合并胰岛素抵抗血瘀阳亢痰浊型的患者,随机分为活血潜阳方联合贝那普利治疗组和贝那普利对照组,均治疗12周;检测各组患者的血压、症状积分、空腹血糖(FPG)、胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、颈动脉内膜中层厚度(IMT)、血管阻力指数(RI)、斑块分级及斑块Crouse积分,通过组间比较进一步验证活血潜阳方的相关临床疗效。结果:活血潜阳方治疗组比对照组更有效降低患者收缩压(P<0.05)、舒张压(P<0.01)、症状积分(P<0.01)、FPG(P<0.05)、FINS(P<0.01)、HOMA-IR(P<0.01)、颈动脉RI(P<0.01)、左侧IMT(P<0.05);而治疗前后两组的颈动脉斑块分级和Crouse积分均无显著差异(P>0.05)。结论:活血潜阳方可改善"血瘀-阳亢-痰浊"型高血压病患者血压、胰岛素抵抗及颈动脉粥样硬化。Objective： To explore the effect of＂ Huoxue Qiangyang Formula＂ in treating hypertension and improving insulin resistance and carotid atherosclerosis. Methods： Patients with hypertension and insulin resistance of blood stasis, yang hyperactivity, turbid phlegm pattern were randomly divided into two groups ： treatment group and control group. The patients in treatment group were treated with ＂Huoxue Qiangyang Formula＂ combined with benazepfil and the patients in control group were treated with benazepril, and both for 12 weeks. We detected the blood pressure（ BP）, clinical symptoms scores（ CSS）, fasting plasma glucose（ FPG）, fasting insulin（ FIN）, insulin resistance index （IRI）, carotid arteries resistance index （RI）, carotid intima-media thickness（ IMT）, plaque classification and Creuse integration. We compared the above indexes between the two groups to observe the effects of ＂ Huoxue Qiangyang Formula＂. Results： The levels of systolic blood pressure （ P 〈0. 05 ）, diastolic blood pressure（ P 〈 0. 01 ）, CSS（ P 〈 0.01 ）, FPG （ P 〈 0. 05 ）, FIN （ P 〈 0. 01 ）, IRI （ P 〈 0.01 ）, carotid arteries RI （ P 〈 0.01 ） and IMT of left common carotid artery （ P 〈 0.05 ） of treatment group were significantly lower than those of the control group ; There were no significant difference of descent of plaque classification and Crouse integration between treatment group and control group （ P 〉 0.05 ）. Conclusion： ＂ Huoxue Qiangyang Formula＂ can improve effect of hypertension, insulin resistance and carotid atheroselerosis in patients with hypertension of blood stasis, yang hyperactivity, turbid phlegm pattern.
