基于MRI针刀治疗冈上肌肌腱炎的临床随机对照研究

摘　　要：目的:观察基于MRI针刀治疗冈上肌肌腱炎的临床疗效。方法:将72例冈上肌肌腱炎患者随机分为治疗组和对照组,每组36例。治疗组采用针刀治疗,对照组采用电针治疗,观察周期为2周。评价两组患者的临床疗效,比较两组患者的疼痛视觉模拟量表(VAS)评分及疼痛弧试验阳性率的变化。结果:治疗后,治疗组的总有效率为91.2%,对照组为84.8%,治疗组的疗效优于对照组(P<0.05)。治疗后,两组患者的VAS评分均显著降低(P<0.01),且治疗组患者的评分低于对照组(P<0.01);两组患者的疼痛弧试验阳性率均显著降低(P<0.01),治疗组患者的疼痛弧试验阳性率为17.65%,对照组为27.27%,但两组比较差异无统计学意义(P>0.05)。结论:基于MRI针刀治疗冈上肌肌腱炎具有较好的疗效,可明显缓解患者的疼痛症状,改善患者的体征。Objective： To observe the curative effect of acupotomy based on MRI in treating supraspinatus tendinitis. Methods： A total of 72 patients with supraspinatus tendinitis were randomly divided into treatment group and control group with 36 eases in each group. The patients in the treatment group were treated with acupotomy and the patients in the control group were treated with electro-acupuncture. The observational course was 2 weeks. The clinical effect was evaluated, and the scores of Visual Analogue Scale （VAS） and the positive rate of pain are were compared between the two groups. Results ： After treatment, the total effective rates of treatment group and control group were 91.2% and 84.8% respectively and the effect of treatment group was better than that of the control group （P 〈 0.05 ）. After treatment, the VAS scores of both groups were obviously decreased （ P 〈0.01 ）, and the scores of the treatment group were lower than those of the control group （ P 〈0.01 ）. After treatment, the positive rates of pain are of both groups were decreased （ P 〈 0.01 ）, and the positive rates of pain are of the treatment group and the control group were 17.65% and 27.27% respectively with no statistical difference between two groups （ P 〉 0.05 ）. Condusion： Aeupotomy based on MRI has good effect in treating supraspinatus tendinitis and can relieve the pain symptom and improve the signs.
