摘 要:目的探讨乌拉地尔在颈从麻醉下行甲状腺手术患者中单用与联用对患者手术期间心血不良反应的防治作用差异。方法收治的行甲状腺次全切手术的患者72例随机分为单用组和联合组,每组36例。单用组采用乌拉地尔0.25 mg/kg,静脉注射,联用组现行静脉注射乌拉地尔0.25 mg/kg后,再缓慢静脉注射美托洛尔0.035 mg/kg,比较2组患者麻醉前后的血流动力学变化情况。结果单用组和联用组患者的基础值、给药前测定的SBP、DBP值差异均无统计学意义(P>0.05),给药后5 min、15 min,2组患者的SBP、DBP值较给药前和基础值均显著的下降(P<0.05),联用组给药后5 min的DBP、15 min的SBP、DBP值均显著低于单用组(P<0.05)。2组患者的基础值、给药前、给药后5 min的心率、平均动脉压测定值差异均无统计学意义(P>0.05),给药后5 min、15 min2组患者的心率、平均动脉压测定值与基础值、给药前测定值差异均有统计学意义(P<0.05),给药后15 min联用组患者的心率、平均动脉压测定值均显著的低于单用组(P<0.05)。2组患者的基础值、给药前、给药后5 min的RPP测定值差异均无统计学意义(P>0.05),给药后15 min联用组RPP测定值显著低于单用组(P<0.05)。结论对颈从麻醉下行甲状腺手术患者采用乌拉地尔联合美托洛尔能够减轻手术过程中的心血管不良反应,稳定血流动力学指标。Objective To compare the preventive effect of urapidil single application and combined application with metoprolol on cadiovascular adverse reaction during thyroid surgery with cervical plexus anesthesia. Methods Seventy-two patients who received thyroid subtotal cutting surgery were randomly divided into single application group and combination application group,with 36 patients in each group. The patients in single application group and combination application group were intravenously injected with urapidil 0. 25 mg / kg,however,after the patients in combination application group were intravenously injected with urapidil 0. 25 mg / kg,who were intravenously injected with metoprolol 0. 035 mg / kg,then the hemodynamic changes before and after anesthesia were compared betweem the two groups. Results There were no significant differences in the base value,SBP,DBP values before medication between two groups( P〉0. 05). SBP,DBP values at 5min,15 min after medication were significantly decreased,as compared with those before medication and base value in both groups( P〈0. 05). The DBP at 5min after medication and SBP,DBP values at 15 min after medication in combination application group were significantly lower than those in single application group( P〈0. 05). There were no siginificant differences in base value,heart rate at 5min before and after medication,mean arterial pressure between two groups( P〉0. 05),however,there were significant differences in heart rate,mean arterial pressure at 5min,15 min after medication,as compared with those before medication and base value( P〈0. 05). Moreover the heart rate and mean arterial pressure at 15 min after medication in combination application group were significantly lower than those in single application group( P〈0. 05).
