魏氏传统手法及蒸敷方治疗腰椎间盘突出症的临床观察

摘　　要：目的:探索魏氏手法联合中药蒸敷治疗腰椎间盘突出症的临床疗效。方法:符合标准的腰椎间盘突出症病人114例,随机分组为联合治疗组(38例)、手法组(38例)及蒸敷组(38例)。记录三组病人治疗前及3周内每天治疗后的VAS评分、Oswestry功能障碍指数及随访2月后总体治疗有效率,对手法及蒸敷单一治疗与两者联合治疗进行疗效评价比较。结果:在第7、14天时,联合治疗组及手法组VAS值下降范围较单纯蒸敷组明显加大,且治疗前后联合组Oswestry功能障碍指数改善优于单一手法组和蒸敷组。2月后随访总体治疗有效率:联合组92.1%,手法组60.5%,蒸敷组73.7%.结论:手法联合蒸敷治疗效果优于单一手法及蒸敷治疗,能有效减轻患者的疼痛程度,改善患者活动功能,中期随访疗效确定,长期疗效有待进一步观察。Objective.. To explore the clinical efficacy of Wei＇s manipulation combined with Chinese fumigation in the treatment of lumbar disc herniation. Methods： 114 patients with lumbar disc herniation were randomly divided into combination treatment group, the manipulation group and the fumigation group, 38 cases in each group. The VAS scores, Oswestry Disability Index before treatment and after daily treatment （for 3 weeks） and the total effective rate were being recorded in three groups. The efficacy of the three groups was compared at the same time. Results： The VAS score at 7 and 14 days were significantly decreased in the combination treatment group and manipulation group compared with the fumigation group. The Oswestry Disability Index before and after treatment in the combination treatment group was better than that in the manipulation group and the fumigation group. The total effective rate in the combination group was 92. 1%0, 60.5% in the manipulation group and 73.7% in the fumigation group. Conclusion： Combined manipulation and fumiga tion treatment is better than single manipulation or fumigation treatment for it can effeetively reduce the pain of patient and improve patient functional activity disorder. The medium-term outcome is determined and the long-term effect needs further observation.
