三点法针刀松解髌下脂肪垫治疗膝关节疼痛的临床研究

摘　　要：目的:观察三点法针刀松解髌下脂肪垫治疗膝关节疼痛的疗效。方法:将120例膝关节疼痛患者随机分为治疗组、针刀对照组、针刺组和西药组,每组30例。治疗组行三点法针刀松解髌下脂肪垫,但不松解其它组织;针刀对照组按照传统痛点定点,只松解病变组织,不松解髌下脂肪垫;针刺组接受针刺治疗;西药组口服双氯芬酸钠缓释胶囊。治疗一个疗程后观察压痛点数、疼痛分级及功能分级的变化。结果:治疗一个疗程后,各组压痛点个数均减少,疼痛分级0、Ⅰ级数及治疗组功能分级0、Ⅰ级数均提高,与治疗前比较,差异具有统计学意义(P<0.05);针刀对照组、针刺组和西药组功能分级0、Ⅰ级数治疗后有所提高,但与治疗前比较无统计学意义(P>0.05);治疗组与针刀对照组、针刺组及西药组比较,治疗组治疗后压痛点数、疼痛分级0、Ⅰ级数和功能分级0、Ⅰ级数变化更明显,差异有统计学意义(P<0.05)。结论:三点法针刀松解髌下脂肪垫能够有效治疗膝关节疼痛。Objective： To observe the curative effect of acupotomy lysis infrapatellar fat pad with three point method to treat knee joint pain. Methods： 120 patients with knee joint pain were randomly divided into treatment group, acupotomy control group, acupuncture group and Western medicine group, 30 patients in each group. Patients in treatment group received acupotomy lysis of infrapatellar fat pad with three point method, not relaxing other tissues. The acupotomy control group received acupotomy lysis of other tissues, not relaxing infrapatellar fat pad. The acupuncture group received acu puncture treatment and the western medicine group were treated with Diclofenac Sodium Sustained Release Capsules orally. The changes of tenderness points, pain classification and functional classification was observed after a course of treat ment. Results： After a course of treatment, the number of tenderness points in each group were reduced. The 0, Ⅰ progression of pain rating and function grade in treatment group were increased. The difference was statistically significant （P〈C0.05）. The 0, Ⅰ progression of pain rating and function grade in acupotomycontrol group, acupuncture group and western medicine group were also increased,but the difference was not statistically significant （P〉0.05）compared with before treatment. The change of 0, Ⅰ progression of pain rating and function grade in treatment group was statistically significant different （P〈0. 05） compared with the acupotomycontrol group, acupuncture group and western medicine group. Conclusion： Acupotomy lysis of infrapatellar fat pad with three point method is effective in treating knee joint pain.
