摘  要:目的 评价氯胺酮对神经病理性痛大鼠脊髓背角NADPH氧化酶2(NOX2)表达的影响.方法 健康雄性SD大鼠36只,6周龄,体重180 ~ 220 g,采用随机数字表法,将其分为3组(n=12):假手术组(S组)、神经病理性痛组(NP组)和氯胺酮组(K组).采用坐骨神经慢性压迫法制备大鼠神经病理性痛模型.术后3d开始K组每天腹腔注射盐酸氯胺酮注射液10 mg/kg至术后21 d;S组和NP组注射等容量生理盐水.分别于术前1d、术后3、7、14和21 d时测定机械缩足反应阈(MWT)和热缩足潜伏期(TWL).于术后7d痛阈测定结束后,每组处死3只大鼠,取L45脊髓背角,采用Western blot法测定NOX2蛋白表达,采用RT-PCR法测定NOX2 mRNA表达.结果 与S组比较,NP组和K组术后3、7、14和21 d时MWT降低,TWL缩短,术后7d时脊髓背角NOX2蛋白及其mRNA表达上调(P<0.05);与NP组比较,K组术后7、14和21 d时MWT升高,TWL延长,术后7d时脊髓背角NOX2蛋白及其mRNA表达下调(P<0.05).结论 氯胺酮减轻大鼠神经病理性痛的机制与其下调脊髓背角NOX2表达有关.Objective To evaluate the effect of ketamine on NOX2 expression in the spinal dorsal horn of rats with neuropathic pain.Methods Thirty-six healthy male Sprague-Dawley rats,aged 6 weeks,weighing 180-220 g,were randomly divided into three groups (n =12 each) using a random number table:sham operation group (S group),neuropathic pain group (NP group) and ketamine group (K group).Neuropathic pain was induced by chronic constrictive injury in chloral hydrate-anesthetized rats.Sciatic nerve was exposed and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals with 4-0 silk thread.In K group,ketamine hydrochloride injection 10 mg/kg was injected intraperitoneally everyday starting from 3 days after operation until 21 days after operation.The equal volume of normal saline was given instead in S and NP groups.At 1 day before operation and 3,7,14 and 21 days after operation,the mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured.Three animals were sacrificed after measurement of the pain threshold at 7 days after operation,the L4,5 segment of the spinal cord was removed for determination of NOX2 expression (by Western blot) and NOX2 mRNA expression (by using real-time PCR) in the spinal dorsal horn.Results Compared with S group,the MWT was significantly decreased,and the TWL was shortened at 3,7,14 and 21 days after operation,and the expression of NOX2 protein and NOX2 mRNA was up-regulated at 7 days after operation in NP and K groups.Compared with NP group,the MWT was significantly increased,and thc TWL was prolonged at 7,14 and 21 days after operation,and the expression of NOX2 protein and NOX2 mRNA was downregulated at 7 days after operation in K group.Conclusion The mechanism by which ketamine mitigates neuropathic pain is related to down-regulation of NOX2 expression in the spinal dorsal horn of rats.
