血液透析联合血液灌流治疗肾性骨营养不良的疗效

摘　　要：目的探讨血液透析联合血液灌流治疗尿毒症维持性血液透析患者肾性骨营养不良的疗效。方法选取维持性血液透析合并肾性骨营养不良的患者30例,随机均分为治疗组和对照组(n=15),治疗组采用血液透析联合血液灌流治疗,对照组采用常规血液透析治疗,2组均每周3次,连续4周,观察2组治疗4周前后肾性骨营养不良症状改善情况及血磷、甲状旁腺激素(i PTH)值的变化。结果治疗组肾性骨营养不良治疗有效率为80%,症状改善情况显著高于对照组,差异有统计学意义(P<0.05),治疗组治疗后血磷值为(2.11±0.32)mmol/L,低于对照组的(3.38±0.39)mmol/L,治疗组甲状旁腺激素治疗后为(450.27±150.62)pg/m L,低于对照组(910.38±299.72)pg/m L,治疗组血磷、甲状旁腺激素降低程度差异显著大于对照组,差异有统计学意义(P<0.05),2组血尿素氮、肌酐差异无统计学意义。结论血液透析联合血液灌流可以明显降低维持性血液透析合并肾性骨营养不良患者的血磷、甲状旁腺激素,改善肾性骨营养不良的临床症状。Objective To observe the effect of hemodialysis combined with hemoperfusion in patients of maintenance hemodialysis with renal osteodystrophy. Methods 30 cases of maintenance hemodialysis with renal osteodystrophy were randomly divided into treatment group and control group, 15 cases in each group, The treatment group was treated with hemodialysis combined with hemoperfusion,the control group was treated with routine hemodialysis, the two groups were treated 3 times a week for 4 weeks, the two groups were observed of improvement of symptoms and serum phosphorus, parathyroid hormone （iPTH） changes before and after treatment. Results The effective rate of renal osteodystrophy in treatment group was 80%, the symptom improvement was significantly higher than that of the control group （P〈0.05）. The serum phosphorus value （2.11±0.32） mmol/L was lower than that in the control group （3.38±0.39） mmol/L, parathyroid hormonethe value（450.27±150.62）pg/mL was lower than that in the control group（910.38±299.72）pg/mL, difference was statistically significant （P〈0.05）, the difference of Blood urea nitrogen and creatinine were not statistically significant between the two groups. Conclusion Hemodialysis combined with hemoperfusion can obviously reduce the maintenance hemodialysis patients with renal osteodystrophy in patients with serum phosphorus, parathyroid hormone, improve the clinical symptoms of renal osteodystrophy.
