健脾清肠汤内服联合愈疡灌肠方保留灌肠治疗慢性持续型溃疡性结肠炎活动期患者31例临床观察

摘　　要：目的观察中药健脾清肠汤内服联合愈疡灌肠方保留灌肠治疗脾虚湿热证慢性持续型溃疡性结肠炎活动期的临床疗效。方法将62例脾虚湿热证慢性持续性溃疡性结肠炎活动期患者随机分为治疗组与对照组各31例,治疗组给予健脾清肠汤内服,并配合愈疡灌肠方保留灌肠治疗;对照组给予口服美沙拉嗪肠溶片每次1 g,每周4次。两组连续治疗12周后进行主要症状评分及镜检结肠黏膜Baron内镜评分以评价临床疗效,并观察不良反应。结果治疗组临床疗效总有效率为93.55%,对照组为83.87%,治疗组优于对照组(P<0.05)。两组治疗后腹痛、腹泻及脓血便症状积分及Baron内镜评分与治疗前比较均明显降低(P<0.01);治疗后治疗组腹痛、腹泻积分及Baron内镜评分均明显低于对照组(P<0.05)。治疗组无不良反应发生,对照组有3例患者出现不良反应。结论健脾清肠汤内服联合愈疡灌肠方保留灌肠治疗脾虚湿热证慢性持续性溃疡性结肠炎活动期疗效肯定,能够改善患者腹痛、腹泻、脓血便等主要临床症状,促进患者肠黏膜愈合,且无明显不良反应。Objective To observe the clinical efficacy of oral administration of traditional Chinese medicine Jianpi Qingchang Decoction（ 健脾清肠汤） combined with retention enema with Yushang Guanchang Decoction（ 愈疡灌肠方）in the treatment of chronic persistent ulcerative colitis with the spleen deficiency and dampness heat syndrome.Methods A total of 62 cases of chronic persistent ulcerative colitis with the spleen deficiency and dampness heat syndrome were randomly divided into treatment group and control group,with 31 cases in each group. The treatment group was given oral administration of Jianpi Qingchang Decoction combined with retention enema with Yushang Guanchang Decoction; The control group was given orally mesalazin enteric-coated tablets 1g each time,4 times a week.After 12 weeks of continuous treatment,the main symptom score and Baron endoscopic score of endoscopic examination of the colon mucosa in both groups were got to evaluate the clinical efficacy,and the adverse reactions were observed. Results The total effective rate was 93. 55% in the treatment group and 83. 87% in the control group,the treatment group was better than the control group（ P〈0. 05）. After treatment,the abdominal pain,diarrhea and bloody purulent stool integral and Baron endoscopic score in both groups were significantly decreased than those before the treatment（ P〈0. 01）; The scores of abdominal pain,diarrhea integral and Baron endoscopic score in the treatment group were significantly lower than those in the control group（ P〈0. 05）. No adverse reactions occurred in the treatment group,there were 3 cases of adverse reactions in the control group. Conclusion Oral administration of traditional Chinese medicine Jianpi Qingchang Decoction combined with retention enema with Yushang Guanchang Decoction in the treatment of chronic persistent ulcerative colitis with the spleen deficiency and dampness heat syndrome do have curative effect,it can improve the main clinical symptoms of patients,such as abdominal
