腰痛宁胶囊治疗腰椎骨性关节炎寒湿瘀阻型72例临床观察

摘　　要：目的观察腰痛宁胶囊治疗腰椎骨性关节炎寒湿瘀阻型的临床疗效及安全性。方法将108例寒湿瘀阻型腰椎骨性关节炎患者随机分为腰痛宁高剂量组、腰痛宁低剂量组和痹痛宁组各36例。腰痛宁高剂量组口服腰痛宁胶囊,每次6粒,每日1次;腰痛宁低剂量组口服腰痛宁胶囊,每次4粒,每日1次;痹痛宁组给予痹痛宁胶囊,每次2粒,每日2次。各组均治疗4周。治疗前及治疗14、28天对患者腰部疼痛程度进行视觉模拟评分法(VAS)及Oswestry功能障碍指数(ODI)评分,评价临床疗效,并进行安全性评价。结果腰痛宁高剂量组临床疗效总有效率为85.71%,腰痛宁低剂量组为83.33%,痹痛宁组为75.00%,腰痛宁高、低剂量组临床疗效明显优于痹痛宁组(P<0.05)。治疗14天,腰痛宁高剂量组ODI积分较本组治疗前明显下降(P<0.05),治疗28天,各组静息痛、活动痛VAS评分及ODI积分均较本组治疗前、治疗14天明显下降(P<0.05);治疗14、28天,与腰痛宁低剂量组、痹痛宁组比较,腰痛宁高剂量组静息痛、活动痛VAS评分均明显降低(P<0.05);治疗28天与腰痛宁低剂量组、痹痛宁组比较,腰痛宁高剂量组ODI积分明显降低(P<0.05)。各组均无明显不良反应。结论腰痛宁胶囊治疗腰椎骨性关节炎寒湿瘀阻型,能够有效缓解患者腰部疼痛,改善腰部功能,且不良反应较少。Objective To observe clinical efficacy and safety of Yaotongning Capsule（ YTN） treating lumbar osteoarthritis of cold-dampness and blood stasis type. Methods Totally 108 patients with lumbar osteoarthritis of cold-dampness and blood stasis type were randomly divided into YTN high dose group,YTN low dose group and Bitongning Capsule（ 痹痛宁胶囊,BTN） group,with 36 cases in each group. The YTN high dose group took 6 YTN every time,once a day. The YTN low dose group took 4 YTN every time,once a day. The BTN group took 2 BTN,2 times per day. All groups were treated for 4 weeks. Before treatment,on the 14 th and the 28 th treated day,visual analogue scale（ VAS） and Oswestry disability index（ ODI） were used to grade degree of lumbar pain of patients. We also evaluated clinical efficacy and safety. Results The total efective rate was 85. 71% in the YTN high dose group,83. 33% in the YTN low dose group and 75. 00% in the BTN group. Clinical efficacy in the YTN high and low dose group was obviously superior to the BTN group. ODI score of the YTN high dose group treated for 14 days significantly decreased in comparison to that before treatment（ P〈0. 05）. VAS score of rest and activity pain and ODI score of each group treated for 28 days significantly decreased in comparison to that before treatment and treated for 14 days（ P〈0. 05）. VAS score of rest and activity pain of the YTN of high dose group treated for 14 and 28 days significantly decreased in comparison to that of the YTN low dose group and the BTN group（ P〈0. 05）. ODI score of the YTN of high dose group treated for 28 days significantly decreased in comparison to that of the YTN low dose group and the BTN group（ P〈0. 05）. Each group had no obvious adverse reactions. Conclusion YTN can treat lumbar osteoarthritis of cold-dampness and blood stasis type by effectively relieving the waist pain of patients and improving the function of the waist,with less adverse reaction.
