摘 要:目的分析HIV抗体不确定样本的血清学特征,结合抗体试验结果,探索HIV抗体不确定样品的分类确证策略。方法以随访最终结果为判断标准,回顾分析和评价免疫印迹试验HIV不确定的结果与ELISA结果间的联系。结果 58例不确定样品中,env类、pol类和gag类带型分别占60.3%、3.4%和36.2%。单一带型中gp160,p24占31.0%;p24占29.3%;gp160占13.8%;gp160,gp41,p24占12.1%;p24,p17占5.2%。40例随访样品中,有5例样品由HIV抗体不确定转归为HIV抗体阳性,均为env类条带的不确定,且两次ELISA检测结果 S/CO值大于6。结论各类不确定结果预示HIV感染的意义不同。单一的gag带和pol带与HIV感染的关系不大,但是同时有env带和gag带(或)pol带HIV感染关系比较密切。随访样品初次检测中,两种试剂检测样品S/CO>6时,HIV-1抗体阳转可能性较大。Objective To explore classification confirmation strategy of indeterminate HIV antibody samples by analyzing the serological characteristics and results of HIV antibody tests. Methods The final follow-up result was taken as the judgment standard,and relation between the results by ELISA and indeterminate results of HIV with Western Blot. Results Among the 58 indeterminate samples,the proportions of env,pol and gag were 60. 3%,3. 4% and 36. 2%,respectively. In the single belt type,Gp160,p24 accounted for 31. 0%,P24 accounted for 29. 3% and Gp160 accounted for 13. 8%,Gp160 gp41,p24 accounted for 12. 1%,and P24,p17 accounted for 5. 2%. In the 40 follow- up cases,5 lapsed to antibody positive from HIV determinate type,and the S / CO values of two ELISA test results were both greater than 6. Conclusion Different determinate results indicate different HIV infection significance.Single gag and pol each matters little to HIV infection,yet combination of env belt and gag with( or) pol with HIV infection implicates more close relation with HIV infection. In test of follow- up samples,S / CO〉6 with both two kinds of reagent test indicates a much higher possibility of seroconversion of HIV- 1 antibody.
