摘  要:目的研究机械通气危重症患儿血清白蛋白水平对临床预后的影响和临床意义。方法以2013年1月-2014年1月该院收治的机械通气危重患儿100例为研究对象,入院24 h内评估小儿危重症评分(PCIS)、检测血清白蛋白水平,行Pearson相关性分析;按血清白蛋白水平,将患儿划分为〉35 g/L组、25-35 g/L组和≤25 g/L组,统计各组多脏器功能障碍综合征(MODS)受累脏器数目、机械通气时间、重症监护室(PICU)入住时间、住院时间、放弃或死亡例数、呼吸机相关性肺炎例数,行q检验及χ2检验。结果血清白蛋白水平与PCIS呈显著正相关(r=0.341,P〈0.001);血清白蛋白水平越高,MODS受累脏器数目越少,机械通气、PICU入住时间、住院时间均越短,放弃或死亡例数、呼吸机相关性肺炎例数均越少,上述项目各组间对比,差异均有统计学意义(P〈0.05)。结论机械通气危重症患儿血清白蛋白水平与临床预后关系显著,检测其浓度有助于预测患儿病情严重程度及治疗成功率。Objective To research the effects of serum albumin levels on clinical prognosis of critically ill children receiving mechanical ventilation and the clinical significance. Methods One hundred critically ill children receiving mechanical ventilation in the hospital from January 2013 to January 2014 were selected,Pediatric Critical Illness Score( PCIS) was used to evaluated the children within 24 hours,serum albumin levels were detected,Pearson correlational analysis was performed; the children were divided into 35 g / L group,25- 35 g / L group,and ≤25 g / L group according to serum albumin levels; the numbers of affected organs of children with multiple organ dysfunction syndrome( MODS),the time of mechanical ventilation,the time in pediatric intensive care unit( PICU),hospitalization time,the numbers of surrender or death,the numbers of children with ventilator- associated pneumonia in the three groups were statistically analyzed; q test and χ^2test were conducted. Results Serum albumin level was positively correlated with PCIS( r = 0. 341,P 〈0. 001); the higher serum albumin level was,the fewer the affected organs of children with MODS were,the shorter the time of mechanical ventilation,in PICU,and hospitalization was,there were statistically significant differences in the above- mentioned indexes among the three groups( P 〈0. 05). Conclusion Serum albumin levels of critically ill children receiving mechanical ventilation are significantly correlated with clinical prognosis,detecting serum albumin levels is helpful to predict the severity of the disease and the successful rate of treatment.
