补肾法对比他汀治疗颈动脉粥样硬化疗效与安全性的系统评价

摘　　要：目的:评价补肾法中医药对比他汀类西药在颈动脉粥样硬化治疗中的疗效及安全性。方法:计算机检索中英文数据库,纳入以补肾法为首要治法的中医药治疗组和以他汀为对照组治疗颈动脉粥样硬化的随机对照试验(RCT),由两名评价者分别独立提取资料、进行方法学质量评价和分析。用Rev Man 5.0软件进行统计分析。结果:共纳入18个随机对照试验。Meta分析结果显示,治疗前两组之间各结局指标均无差异;治疗后两组颈动脉IMT、颈动脉斑块crouse积分、TC、LDL-C、HDL-C及不良反应发生率无统计学差异。治疗组较他汀对照组,降低TG值更明显,中医症候疗效更高,随访心脑血管事件发生率更低。结论:补肾法中医药较他汀类治疗颈动脉粥样硬化中医症候疗效率更高、降低TG值更明显,并能减少心脑血管事件的发生。由于纳入的研究存在选择性、实施、测量性偏倚的高度可能性,尚需更多的高质量研究提供可靠的证据对其疗效及安全性予以证实。Objective： To evaluate the efficacy and safety of method of supplementing kidney for carotid atherosclerosis compared with statins. Methods： The randomized controlled trials（RCT） on method of supplementing kidney versus statins for carotid atherosclerosis were searched. Two reviewers independently selected the research,extracted data and assessed the methodological quality. And then the data was carried out for the Meta-analysis by Rev Man 5. 0 software. Results： A total of 18 RCTs involving 1885 patients were included. The results of Meta-analysis showedthere was no difference between two groups on basic information before treatment. After treatment of statins or method of supplementing kidney,there was no difference on IMT,crouse scores of plaques,TC,LDL-C,HDL-C and the adverse effects rate. Compared with the treatment of control group,the treatment group decreased TG more significantly and improved the TCM syndrome and the follow-up occurrence rate of the heart and cerebrum events was lower. Conclusion： The method of supplementing kidney is better than statins for patients with carotid atherosis on improving TCM syndrome,decreasing TG and the occurrence of cardiovascular events. More high quality RCTs are required to further verify this conclusion.
