应用连续血液净化抢救糖尿病乳酸酸中毒25例体会

摘　　要：目的：探讨应用连续血液净化对糖尿病乳酸酸中毒患者进行抢救治疗的疗效。方法回顾性分析2007年1月-2011年12月共25例糖尿病乳酸酸中毒病例的临床资料。25例分为常规组10例和血液滤过组15例，监测两组患者治疗前后血乳酸、动脉血气，比较两组患者乳酸清除的效果及死亡率。结果两组患者血pH值上升水平、血乳酸下降水平间存在显著性差异（P 〈0.05），血液滤过组优于常规组；两组病死率之间存在显著性差异（P 〈0.05）。结论连续血液净化对于糖尿病乳酸性酸中毒患者是很好的治疗手段。Objective To investigate the therapeutic effect of continuous blood purification in the treatment of diabetic patients with lactic acidosis.MethodsThe clinical data of 25 diabetic patients with lactic acidosis from January 2007 to December 2011 were retrospectively analyzed.25 patients were divided into 10 patients in the conventional group and 15 patients in the blood filtration group. Blood lactate and arterial blood gas in the two groups were monitored before and after treatment,and the effect of lactate clearance and mortality in the two groups were compared. ResultsThere were significant differences in the blood pH value increased level and the blood lactate decreased level between the patients of two groups（P 〈 0.05）, and the blood filtration group was better than that in the conventional group;the difference was significant in mortality between the two groups（P 〈 0.05）.ConclusionContinuous blood purification is a good method for the treatment of diabetic patients with lactic acidosis.
