摘  要:目的:观察电针对胶原诱导性关节炎(collagen-induced arthritis,CIA)大鼠的镇痛作用及其对炎性反应和滑膜组织内源性阿片系统的调控作用。方法:将30只健康雄性Wistar大鼠随机分为对照组、模型组和电针组,每组10只。模型组和电针组采用牛Ⅱ型胶原建立大鼠CIA慢性痛模型。电针组从造模第16d开始电针干预,穴取双侧"足三里""昆仑",每次30 min,每天1次,连续10d;对照组不予以干预;模型组仅给予同电针组相同的固定。观察造模前、造模第16、20、23、25d各组大鼠痛阈、关节炎指数、足跖肿胀度变化;采用放射免疫法检测大鼠滑膜组织β-内啡肽(beta-endorphin,β-END)、甲硫脑啡肽(met-enkephalin,met-ENK)、强啡肽A(dynorphin A,Dyn A)的含量;采用实时荧光定量PCR法检测大鼠滑膜组织μ阿片受体(mu opioid receptor,MOR)、κ阿片受体(kappa opioid receptor,KOR)、σ阿片受体(delta opioid receptor,DOR)mRNA的表达水平。结果:在造模第16、20、23、25d,模型组大鼠痛阈较对照组降低(均P<0.01),关节炎指数升高(均P<0.01),足跖肿胀度升高(均P<0.01);在造模第23d、25d,电针组较模型组痛阈增加(均P<0.01),关节炎指数和足跖肿胀度降低(均P<0.01)。与对照组比较,模型组大鼠滑膜组织Dyn A含量升高(P<0.01),MOR、KOR、DOR mRNA表达均降至正常水平0.5倍以下;与模型组比较,电针组大鼠膝关节滑膜组织β-END含量增加(P<0.05),均2倍以上上调MOR、KOR DOR mRNA表达水平。结论:电针对CIA大鼠慢性痛有良好的干预作用,可能与电针发挥抗炎和上调滑膜β-END和MOR、KOR、DOR表达水平有关。Objective To observe the analgesic effect of electroacupuncture(EA)on collagen-induced arthritis(CIA)rats and its regulating effect on inflammation reaction and the endogenous opioid system of synovial tissues.Methods A total of 30 healthy male Wistar rats were randomly divided into a control group,a model group and an EA group,10 rats in each one.The chronic pain model of CIA rats was made by cattle type-Ⅱ collagen in the model group and EA group.Rats in the EA group were treated with EA at"Zusanli"(ST 36)and "Kunlun"(BL 60)for 30 min from 16 th day after model establishment,once a day for consecutive 10 days.Rats in the control group did not receive any treatment.Rats in the model group were treated with fixation as the EA group.Threshold of pain,arthritis index,paw swelling were measured before model establishment and 16 d,20d,23 d and 25 dafter model establishment.The levels of beta-endorphin(β-END),met-enkephalin(met-ENK),dynorphin A(Dyn A)were measured by radioimmunoassay;the mRNA expressions of mu opioid receptor(MOR),kappa opioid receptor(KOR)and delta opioid receptor(DOR)in synovial tissues of CIA rats were detected by quantitative polymerase chain reaction(qPCR).Results Compared with the control group,threshold of pain was reduced(allP〈0.01),arthritis index was increased(allP〈0.01)and paw swelling was increased(allP〈0.01)in the model group on the 16 th day,20 th day,23 rd day,25 th day after model establishment.Compared with the model group,the threshold of pain was increased in the EA group(allP〈0.01),arthritis index and paw swelling were reduced(allP〈0.01)on the 23 rd day and 25 th day after model establishment.Compared with the control group,the level of Dyn A in synovial tissues of CIA rats was increased in the model group(P〈0.01);the mRNA expressions of MOR,KOR and DOR were down-regulated lower than 0.5fold of normal level.Compared with the model group,the level ofβ-END in synovial tissues of the knee joint
