摘  要:目的探讨化学发光免疫分析法(CLIA)中不同阶段的测量值在梅毒诊断中的应用。方法收集2013至2015年门诊、术前患者及单位体检者经化学发光法检测的梅毒螺旋体抗体阳性标本500例,并且记录其S/CO值,阳性结果(S/CO≥1)以梅毒螺旋体明胶颗粒凝集法(TPPA)为确证试验进行确认,甲苯胺红不加热血清学试验(TRUST)为辅助试验检测梅毒现症感染,分析化学发光免疫分析法(CLIA)不同阶段的测量值的真阳性率及现症感染率。结果通过TPPA作为确证试验可以得出,500份梅毒螺旋体抗体阳性标本中真阳性标本429例,真阳性率85.8%,其中S/CO值1~2之间真阳性率为20.83%,2—3之间的真阳性率为52.63%,3~5之间的真阳性率为85.71%,5~7之间的真阳性率为96.77%,〉7的真阳性率为100%;通过甲苯胺红不加热血清学试验(TRUST)筛查可以得出,500份梅毒螺旋体抗体阳性标本中现症感染率为44%,其中S/CO值1~3之间现症感染率为0,3~5之间现症感染率为3.57%,5.7之间现症感染率为6.45%,7~10之间现症感染率为13.16%,〉10的现症感染率为67.94%。结论CLIA法检测梅毒,虽然实现自动化,但仍存在一定的假阳性,不能单凭其结果阳性就判断其感染梅毒,必要时需结合TPPA和TRUST法进行补充试验,排除假阳性和现症感染。Objective To explore the measured value of chemiluminescence immunoassay (CLIA) in differenphase in the diagnosiof syphilis. Method500 caseof treponempallidum antibody positive specimendetected by CLIfrom outpatient, preoperative patientand physical examineewere collected from 2013 to 2015, and recorded the S/CO value, positive result(S/CO ≥1) were tested by treponempallidum gelatin particle agglutination (TPPA) athe corroboration tesvalidation, tested currensyphiliinfection by toluidine red unheated serological tes(TRUST) athe auxiliary test, analyzed the true positive rate and the curreninfection rate of measured value of CLIadifferenstages. ResultTPPtesshowed, there were 429 true positive specimenin all 500 specimens, with the true positive rate of 85.8%, the true positive rate wa20.83% when S/CO value wa1-2, 52.63% of S/CO value 2-3, 85.71% of S/CO value 3-5, 96.77% of S/CO value 5-7, 100% of S/CO value 〉7. TRUSscreening showed, the curreninfection rate in 500 specimenwa44%, the curreninfection rate wa0% when S/CO value wa1-3, 3.57% of S/CO value 3-5, 6.45% of S/CO value 5-7, 13.16% of S/CO value 7-10, 67.94% of S/CO value 〉 10. ConclusionThere are still false positive errorin CLImethod to detecsyphilialthough automation, so we cannodiagnose syphiliinfection only by itpositive result. Combination with TPPand TRUSmethod ihelpful to eliminating false positive errorand curreninfection.
