摘 要:目的探讨甲状腺实性结节的超声造影增强模式与CD34、Galectin-3的表达在良恶性结节中差异及两者相关性。方法甲状腺结节患者113例,共117个实性结节,其中良性79个,恶性38个,术前行超声造影,术后行CD34、Galectin-3免疫组织化学染色。比较良恶性结节超声造影增强模式和免疫组织化学表达水平差异,分析其相关性。结果超声造影示甲状腺良性结节77.2%(61/79)表现为环状增强,恶性结节81.6%(31/38)表现为不均匀增强,二者差异有统计学意义(P<0.01)。恶性结节CD34、Galectin-3表达高于良性结节(P<0.01)。非均匀增强与CD34、Galectin-3呈正相关(r=0.41、0.29,P<0.01);环状增强与CD34、Galectin-3呈负相关(r=-0.32、-0.48,P<0.05)。结论超声造影增强模式与CD34、Galectin-3存在相关性,有助于鉴别甲状腺实性结节的良恶性。Objective To investigate the correlation of enhancement patterns from thyroid solid nodules by contrast enhanced ultrasound (CEUS) and CD34, Galectin-3 expression. Methods There were 117 solid nodules in 113 patients, including 79 benign and 38 malignant nodules. The differences in enhancement patterns and expression level of immune tissues between benign and malignant nodules were compared and the correlation between them was analyzed. Results As showed by CEUS,77.2%(61/79) benign nodules manifested ring enhancement, but 81.6%(31/38) malignant nodules showed inhomogeneous enhancement, and the difference was significant (P 〈0.01).The expressions of CD34, Galectin-3 were significantly higher in malignant nodules than those in benign ones (P〈0.01).Inhomogeneous enhancement was positively correlated with CD34, Galectin-3 (r=0.41,0.29, P〈0.01 ).Ring enhancement was negatively correlated with CD34, Galectin-3 (r=-0.32,-0.48,P〈0.05). Conclusion CEUS and immunohistochemistry are both useful tools to differentiate benign and malignant thyroid nodules.
