摘　　要：目的了解北京部分地区被调查老年人群中慢洼阻塞性肺疾病（chronic obstructive pulmonary diseases, COPD）患病率及危险因素。方法采用统一的表格对研究对象行问卷调查，并进行肺功能检测及简要体格检查。结果355例老年研究对象中，共检出COPD患者62例，总患病率为17．5％，其中男性患病率为21．8％，女性患病率为12．3％，男性高于女性（P〈0．05）。随年龄增高患病率逐渐上升。单因素分析提示性别、年龄、家族哮喘史、家族COPD史、吸烟史、饮酒史、每周体育运动次数、居住情况和反复呼吸道感染等九项与老年人群COPD患病有关。多因素Logistic回归分析发现，年龄增长、COPD家族史、吸烟、反复呼吸道感染是老年人群患COPD的独立危险因素。结论老年人群COPD患病率高，应针对危险因素采取综合干预措施以控制COPD的发生和进展。Objective To explore the prevalence of chronic obstructive pulmonary disease （COPD） and its risk factors in elder population of this survey in parts of Beijing. Methods A survey of 355 old people in Beijing was performed using uniform scheme, procedures and questionnaire. Physical examinations and Lung function tests were also conducted for every participant. Results In this survey, 62 COPD cases were identified with overall prevalence 17.5%. The prevalence for male was 21.8% while for female it was 12.3%. The prevalence was higher in male than in female （P〈0.05）, and it is getting higher along with age increasing. Factors associated with COPD were identified in one way variance model as follows： gender, age, family history of asthma and COPD, smoking, alcohol consumption, frequency of physical exercise weekly, living conditions and recurrent respiratory infection. Results from multivariate logistic regression analysis showed the independent risk factors for COPD of old population were ageing, family history of COPD, smoking and recurrent respiratory infection. Conclusion The prevalence of COPD of elderly population was high, suggesting that comprehensive intervention measures especially on risk factors controlling should be taken to prevent the development of COPD.
