摘  要:目的:探讨颅内动脉瘤栓塞术中DynaCT(数字平板C型臂血管造影系统CT成像技术)应用的影像学特征、临床使用方式、及对治疗方法的影响,以提高诊断和治疗的水平。方法我院2012年1月至2014年12月颅内破裂动脉瘤栓塞术中使用DynaCT技术的病例65例,所有患者手术后均行DynaCT扫描并与移动螺旋CT对照评估。结果患者年龄45~78岁,平均55岁,男∶女性别比为1∶1.5。经DynaCT扫描并经移动螺旋CT证实蛛网膜下腔出血、脑内血肿、脑室内出血、脑积水等进行对比分析,其中对16例支架辅助动脉瘤栓塞患者应用DynaCT 评估支架打开情况。结论DynaCT在颅内动脉瘤栓塞术中有独特的优势,但也有其缺点。本研究认为DynaCT对蛛网膜下腔出血等诊断足够清晰,特别是支架辅助栓塞动脉瘤中对支架打开情况的评估,对栓塞策略的采取有指导作用。Objective The characteristics of C-arm flat-detector computed tomography ( DynaCT ) in endovascular embolization of intracranial aneurysms are investigated to improve the level of diagnoses and treatments. Methods A total of 65 cases of patients using DynaCT in ruptured aneurysms during endovascular embolization, who admitted to Bayi Brain Hospital from Jan 2012 to Dec 2014 were reviewed and the comparison between DynaCT and spiral CT using for the postoperatively evaluation was made.Results Of the 65 patients, the age ranged from 45 to 78 years ( average, 55 years ) and the rate of male and female was 11∶1.5.The results, such as subarachnoid hemorrhage, intracerebral hematoma, intraventricular hemorrhage, hydrocephalus and so on, scanning by DynaCT and confirming by spiral CT were analyzed, and DynaCT scanning was employed to evaluate the real-time situation of stent implantation in endovascular embolization of 16 patients.Conclusion The DynaCT using in endovascular embolization has the advantages, and also the shortages.This study shows that DynaCT is proper for diagnosing of subarachnoid hemorrhage, especially for the assessment of stent implantation, and is guidable for the endovascular embolization treatment of intracranial aneurysm.
