233例儿童鼻后滴漏综合征临床分析

摘　　要：目的分析儿童鼻后滴漏综合征的病因、诊断及综合疗法的临床疗效。方法回顾性分析符合鼻后滴漏综合征诊断标准儿童233例,均采用糖皮质激素雾化吸入、阿莫西林联合克拉维酸钾、标准桃金娘油、通窍鼻炎片口服治疗。疗后定期复诊随访1年,分析治疗方法及疾病转归情况,总结诊治经验。结果 233例患儿中,发生误诊、漏诊者共161例;采用本治疗方案2周治愈128例(54.9%),3周治愈184例(79.0%),4周治愈202例(86.7%),9例(3.9%)显效,5例(2.1%)有效,17例(7.3%)无效;1年后复发10例。结论儿童鼻后滴漏综合征易被误诊误治,给予糖皮质激素雾化吸入、阿莫西林克拉维酸钾、标准桃金娘油、通窍鼻炎片治疗,可有效消除继发性咳嗽,临床疗效显著。Objective To investigate the causes and diagnostic strategy for postnasal drip syndrome in children. Methods A retrospective analysis was made among 233 cases of children with postnasal drip syndrome. They were given a combined therapy composed of glucocorticoid inhalation and orally taken amoxicillin clavulanate potassium, standard myrtle oil and Tongqiao rhinitis tablets. A period of following up was carried out among them lasted for one year after the end of therapy. Then, clinical data were carefully analyzed to sum up their response to the therapy and our clinical experience with such a condition. Results In this group of cases, 161 had been misdiagnosed before coming to our Hospital. Following the treatment with our combining therapy, 128 cases（54.9%） were cured within a therapeutic course lasted for 2 weeks, 184（79.0%） were cured within a course of 3 weeks, 202（86.7%）cured within 4 weeks, 9（3.9%） markedly improved, 5（2.1%） effective, and only 17 cases（7.3%） ineffective. By the end of following up period, 10 cases were seen recurrenced. Conclusions Postnasal drip syndrome among children is frequently misdiagnosed and un-appropriately treated. Actually, it can be cured by such a combined therapy composed of glucocorticoid inhalation, amoxicillin clavulanate potassium, standard myrtle oil and Tongqiao rhinitis tablets with chronic cough disappeared and clinical therapeutic effect significant.
