摘 要:目的回顾性对比分析椎体成形术(PVP)及椎体后凸成形术(PKP)治疗脊柱压缩性骨折的中期临床疗效,分析术中骨水泥用量与术后椎体高度及疼痛评分的相关性。方法选择2008~2012年胸腰椎压缩性骨折患者共372例,共472个椎体,按术式不同,分为PVP及PKP两组。对472个椎体的术中骨水泥注射量与术后椎体高度,及骨水泥注射量与术后疼痛VAS评分的相关性进行了分析,同时对比了两组患者的骨水泥渗漏发生率及术后2年的椎体高度、疼痛VAS评分。结果患者的骨水泥注射量与术后的椎体高度没有明确的相关性;骨水泥注射量与术后第2天VAS评分没有明确相关性;术后2年椎体COBB角及VAS,PVP组要高于PKP组;骨水泥渗漏发生率两组无差异;骨水泥渗漏多发生于椎体前方。结论行PVP或PKP手术患者术中所注射的骨水泥量与术后的椎体高度和术后的VAS评分并不相关,这对我们在术中把握骨水泥注射量有所帮助。术后2年,单从椎体COBB角和疼痛VAS评分。PKP组要更有优势。Objective To explore the correlation of the intra - operative dosage of bone cement with post - operative vertebral height and pain scores of percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) in the treatment of spine compression fractures. Methods 372 cases with compression fractures of the tho- racic and lumbar spine (a total of 472 vertebral bodies) were divided into the PVP group and the PKP group. The correlation of the intra - operative dosage of bone cement with post - operative vertebral height and postoperative pain VAS score of 472 vertebral bodies were analyzed. The two groups were compared in terms of incidence of cement leakage, vertebral height and pain VAS score 2 years after surgery. Results The correlation of the dosage of bone cement with the post - operative vertebral height in both groups was insignificantly different, and so was it with the re- lationship between the dosage of bone cement and pain VAS scores. The COBB angle of vertebral body and pain VAS 2 years after surgery in the PVP group were higher than those in the PKP group. There was no difference in the inci- dence of bone cement leakage between the two groups, which mostly occurred in front of the vertebral body. Conclu- sion There are no significant correlation between the intra - operative dosage of bone cement and post - operative vertebral height, and pain scores in patients with PVP or PKP. From vertebral COBB and VAS score 2 years after sur- gery, patients with PKP have an advantage over those with PVP.
