老年阻塞性睡眠呼吸暂停低通气综合征患者工作记忆与焦虑情绪及血浆低氧诱导因子1水平的研究

摘　　要：目的了解老年阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者工作记忆与焦虑情绪状况,探讨两者与血浆低氧诱导因子1(HIF-1)水平变化的关系。方法选取2010年6月至2013年6月河北联合大学附属医院确诊为老年OSAHS患者121例作为OSAHS组,另外选择同期健康体检的老年人(排除OSAHS疾病)120例为对照组。分别采用修订的韦氏记忆量表(WMS-CR)、酶联免疫吸附法及焦虑自评量表(SAS)对两组患者的工作记忆情况、HIF-1水平及焦虑情况进行评价。结果OSAHS组工作记忆各项评分、SAS评分低于对照组,血浆HIF-1水平高于对照组,差异有统计学意义(P<0.01);OSAHS组焦虑发生率显著高于对照组(P<0.01);OSAHS组患者焦虑评分与WMS-CR各项评分呈负相关,HIF-1水平与WMS-CR各评分呈负相关(P<0.05);OSAHS患者睡眠呼吸暂停低通气指数与WMS-CR各项评分呈负相关,与血浆HIF-1水平呈正相关(P<0.05)。结论 OSAHS患者可使工作记忆受到损伤,产生焦虑情绪,伴有血清HIF-1水平的升高;OSAHS患者的工作记忆损伤与HIF-1水平和焦虑情绪相关。Objective To assess the anxiety and working memory function in elderly patients with obstructive sleep apnea hypopnea syndrome（OSAHS） and to study their relationship with hypoxia-inducible factor-1 （ HIF-1 ）. Methods Total of 121 patients with OSAHS admitted to the Affiliated Hospital of Hebei United Univemity from Jun. 2010 to Jun. 2013 were included as OSAHS group, and 120 non-OSAHS patients as control group（ exclusion of OSAHS） for the study. A set of Wechsler memory scale（WMS-CR） , enzyme- linked immunosorbent assay（ELISA） and self-rating anxiety scale （SAS） were used to evaluate the working memory function, anxiety condition and the level of HIF-1 of the two groups. Results The score of working memory,SAS score of OSAHS group were lower than the control group,while the level of HIF-1 was higher than the control group（P 〈0.01 ）;the incidence rate of anxiety of OSAHS group was higher than the control group （ P 〈 0. 01 ）. There were significant negatiye correlation between the score of SAS and the each score of WMS-CR（P 〈 0. 05 ）. There was significant negative correlation between the level of HIF-1 and WMS-CR scores （P 〈 0. 05 ）. There were significant negative correlation between the AHI of OSAHS and each score of WMS-CR, and there was positive correlation between the AHI and the level of HIF-1 （P 〈 0. 05 ）. Conclusion Patients with OSAHS are prone to have working memory and mood impairments with higher plasma level of HIF-1. The working memory impairment has association with HIF-1 and anxiety.
