摘  要:目的探讨MRI对胎儿侧脑室扩张(ventriculomegaly,VM)的临床应用价值。方法回顾性分析96例行胎儿颅脑超声检查,并于3天内行胎儿头颅MRI检查的影像资料,比较MRI及超声在常规平面上(MRI冠状位与超声轴位)测量侧脑室宽度有无扩张的一致性;以产后随访或引产标本为诊断标准,评价二者的诊断价值。结果MRI冠状面测量胎儿VM与超声测量结果有很好的一致性(K=0.835)。MRI确定诊断17例超声疑似诊断,补充诊断5例,更正1例超声诊断结果。结论MRI在诊断胎儿VM及明确有无合并中枢神经系统畸形方面有较高的临床应用价值。Objective To evaluate the application of MRI to diagnose fetal ventriculomegaly (VM). Methods Clinical data were collected from 96 pregnant women who underwent fetal uhrasonography followed by cranial MRI exami- nation within 3 days before retrospective analysis. Then MRI and ultrasonography were compared for their measurement of lateral ventriculomegaly at routine planes (the axial plane for ultrasonography versus the coronal plane for MRI). Data from postpartum follow - up visits or labor specimen were set as diagnostic criteria to evaluate the application of MRI and ultrasound for diagnosis of fetal VM. Results The measurement of MRI at the coronal plane for fetal VM was well con- sistent with that of ultrasonography ( K = 0. 835). According to MRI results, 17 suspected and 5 normal cases by ultra- sonography were diagnosed, while uhrasonographical results were corrected in 1 case. Conclusion MRI examination is highly useful to diagnose fetal VM and determine the presence of abnormalities in the central nervous system.
