中西医结合治疗非特异性间质性肺炎1例报告

摘　　要：目的观察泼尼松配伍银杏叶、雷公藤、百合固金丸、黄芪等中药对非特异性间质性肺炎(NSIP)的疗效。方法运用糖皮质激素(泼尼松起始剂量60 mg/日,逐渐减至10 mg/隔日)配伍中药(银杏叶胶囊0.6g/日、雷公藤多甙30 mg/日、百合固金丸2丸/日和黄芪20 g/日)联合治疗NSIP患者1例,连续应药8 W,分别于给药前后做动脉血气分析、血沉(ESR)、血液生化检测和肺部CT检查。结果给药后患者症状和体征积分比给药前有所降低,动脉血氧分压从67.5 mm Hg升至78.6 mm Hg,ESR从75 mm/h降至19 mm/h,肺CT检查见双肺中下野弥漫性浸润性阴影大部消失,仅余右下肺点状阴影1处。结论用泼尼松配伍银杏叶、雷公藤、百合固金丸、黄芪中西医结合的办法治疗NSIP,有较好疗效。Objective To observe the effect of combination of predison, ginkgo leaf, tripterygium, wilfordii, lilium compound antiphlogistic pill, and astragalus mongholicus on nonspecific interstitial pneumonia. Method To use glucocorticoid（initially prednison60mg/d, gradually reduced to 10 mg every other day） combined with traditional Chinese medicine（ginkgo leaf capsule 0.6g/d, tripterygium 30mg/d, lilium compound antiphlogistic pill 2/d, astragalus mongholicus 20g/d） to treat one case with NSIP, do analysis of arterial blood, ESR, thrombus biochemical detection and lung CT before and after taking the medicine. Result After taking the medicine, the smptoms of the patient got fewer. The arterial partial pressure of oxygen rose from 67.5mm Hg to 78.6mm Hg, ESR decreased from75mm/h to 19mm/h, lung CT showed much of the diffuse infiltrative shadow disappeared, only the shadow on the bottom right still existed. Conclusion The combination of predison, ginkgo leaf, tripterygium, wilfordii, lilium compound antiphlogistic pill, and astragalus mongholicus on nonspecific interstitial pneumonia has good effect.
