α1受体阻滞剂及消脱止-M在治疗双J管相关症状中的应用

摘　　要：目的 评估α1受体阻滞剂及消脱止-M在防治双J管相关症状的作用.方法 制定纳入与排除标准,将符合纳入标准的105例输尿管镜激光碎石术后患者随机分成对照组（n=35）,α1受体阻滞剂组（n=36）,消脱止-M组（n=34）.对照组术后不做任何药物干预;其余两组每天分别口服α1受体阻滞剂组（盐酸坦洛新缓释片,0.2mg,每晚一次）、消脱止-M（500mg,一天3次,一次两粒）,3组患者均在第二周末拔除体内双J管,拔管前完成IPSS症状学评分、长海痛尺疼痛评分问卷检查及尿常规检查.结果 3组患者术前一般情况差异无统计学意义.α1受体阻滞剂组及消脱止-M组在IPSS症状学评分、疼痛评分要明显优于对照组（P＜0.05）,但两者间无明显差异.两实验组的血尿程度明显低于对照组（P＜0.05）,且消脱止-M组血尿程度明显低于α1受体阻滞剂组（P＜0.05）.结论 α1受体阻滞剂和消脱止-M在减轻输尿管镜激光碎石后双J管相关症状中均有明显作用,消脱止-M减轻血尿效果优于α1受体阻滞剂。 .Objectives To evaluate the tamsulosin and melilotus extract tablets in improving double-J stent related urinary tract symptom.Methods Formulate the included and excluded standards,105 patients who fit the included standards with double-J stent placement following ureteroscopic lithotripsy randomize into three groups,the control group（n =35）,the alpha1-receptor blockers group（n =36） and the melilotus group（n =34）.The control group have regular treatment.The other two groups respectively received tamsulosin 0.2mg once daily and melilotus extract tablets 1000mg t.i.d.All patients removed double-J stent in two weeks,and completed the questionnaire（compare IPSS symptoms scores,Changhai pain rating scale for pain assessmentand urine routine examination） before patients removing double-J stent to measure the degree of hematuria.Results No statistical significance between three groups before the surgery.The IPSS,pain scale and the degree of hematuria in the alpha1-receptor blockers group and the melilotus group is significantly better than the control group（P 〈 0.05）,but no significant differences between the alpha1-receptor blockers group and the melilotus group.The degree of hematuria in two experiment groups is significantly lower than the control group,and the degree of hematuria in the melilotus group is significantly lower than the alpha1-receptor blockers group（P 〈 0.05）.Conclusions Tamsulosin and melilotus extract tablets both can improve double-J stent related urinary tract symptom,and the melilotus is better than the tamsulosin in the aspect of reducing the degree of hematuria.
