摘  要:目前,应用功能性电刺激(functional electrical stimulation,FES)进行中风后康复治疗已经十分普遍,但是对于FES治疗过程中大脑的状态变化却缺乏深入的研究.掌握这一变化规律有助于了解大脑在FES治疗过程中的康复机制.笔者尝试通过研究FES诱发的脑电信号,探究在治疗过程中大脑的脑电特征变化.结合FES设计了主动、被动模式(人工被动和刺激下被动)下的下肢运动实验,采集了6名受试者(4名中风患者和2名健康受试者)的数据,并利用时频分析和源定位等方法对数据进行分析.结果发现在多种实验模式下正常受试者与患者的脑电信号中均存在事件相关去同步(event-related desynchronization,ERD)现象,证实了中风患者受损后的大脑区域仍然能够被激活;在电刺激模式下ERD的强度随电刺激的增强而增大,表明适当的高强度电刺激可以为大脑提供更好的激活作用;通过源定位分析发现感觉运动区的激活在动作期较强,而在动作保持期内激活效应较差.研究结果将有助于加深认识FES对大脑的激励作用,同时为医师设计康复治疗方案提供可靠依据.Functional electrical stimulation(FES) is a common way for stroke patients to rehabilitate. the study of changes in brain during FES rehabilitation is limited and this study may help us understand the brain recovery mechanism under FES. This paper aims to study electroencephalography (EEG) features induced by FES to figure out how FES influences the brain. A couple of experimental models of leg movement were designed under active and passive conditions (by man-made and by electrical stimuli). EEG data were collected in each experimental model. The study involved 4 stroke patients and 2 healthy subjects. Methods like time-frequency analysis and source localization analysis were used to process data. The results show that stroke patients, either under FES or passive condition, could also generate event-related desynchronization(ERD), which means the stoke patients' brain may be activated as well; the intensity of ERD increased as the intensity of electrical stimuli increased, which means current with suitable intensity can offer patients with better treatment effect; source localization analysis show that activation of sensory motor areas was stronger when the action was under execution, then turned weaker when the action was over. The results of this study helps deepen the understanding of incentives of FES on the brain and offer reliable theoretical foundation when optimizing the therapeutic schedule.
