血尿酸与主动脉夹层关系的Meta分析

摘　　要：目的综合评价血尿酸水平与主动脉扩张和主动脉夹层的关系,为主动脉夹层的预防和诊治提供依据。方法利用计算机检索中国期刊网全文数据库CNKI、万方数据库、维普数据库和Pub Med、Embase(1995年1月-2014年12月),收集国内外公开发表的有关血尿酸水平与主动脉扩张和主动脉夹层的病例对照研究,采用Rev Man 5.2软件进行分析。结果共纳入12项研究,共4 043例研究对象。Meta分析结果显示,血尿酸与主动脉扩张和主动脉夹层总体效应检验Z=2.23,P=0.03[WMD=60.32,95%CI(7.36,113.28)]。血尿酸与亚组主动脉扩张之间差异有统计学意义[WMD=60.35,95%CI(25.57,95.14),P=0.0007],但与主动脉夹层之间差异无统计学意义[WMD=58.39,95%CI(-21.24,138.01),P=0.15]。总的失安全系数Nfs0.05=2 866,提示Meta分析结果可靠。结论高尿酸与主动脉扩张关系更为密切,高尿酸与主动脉夹层的关系可能受到其他危险因素的交互影响。【Objective】 To evaluate the relationship between serum uric acid（UA） and aortic aneurysm（AA） or aortic dissection（AD） with an attempt to provide evidences for the prevention and treatment of AA and AD. 【Methods】 Major online database including CNKI, Wan Fang, VIP, Pub Med and Embase were searched for a period between January 1995 and December 2014 to collect case-control studies on the association between UA and AA or AD. The data were analyzed using Rev Man 5.2 software and the potential bias was assessed by sensitivity analysis and fail-safe number. 【Results】 A total of 12 studies containing4,043 subjects were enrolled. Meta analysis showed that the overall effect value was Z = 2.23,（P = 0.03） and the weighted mean difference（WMD） value was 60.32 with 95%CI（7.36, 113.28）. In terms of subgroup analysis, SUA levels in the AA groups were remarkably higher than those of the control groups [WMD = 60.35,95%CI（25.57, 95.14）, P = 0.0007], whereas there was no significant difference between the AD and the control groups [WMD = 58.39, 95%CI（-21.24, 138.01）, P = 0.15]. The loss of safety factor Nfs 0.05 of 2,866 indicated that the results of Meta analysis had a good reliability. 【Conclusions】 High serum UA is closely related with AA, and the relationship between UA and AD may be affected by the interaction of other risk factors.
