半髋关节置换与锁定钢板治疗老年股骨粗隆间骨折的疗效比较

摘　　要：目的分析半髋关节置换与锁定钢板治疗老年股骨粗隆间骨折的疗效差异。方法选取老年股骨粗隆间骨折患者52例,随机采用半髋关节置换术与锁定钢板内固定术进行手术治疗,对两组患者的手术时间、术中出血量、术后并发症发生率、术后卧床时间及术后1年髋关节功能进行比较。结果所有患者得到随访。半髋关节置换组手术时间和术后卧床时间短,术中出血量及术后并发症少,与锁定钢板组比较差异有统计学意义(P<0.05);术后1年Harris评分,两组之间差异无统计学意义(P>0.05)。结论对于老年股骨粗隆间骨折患者,半髋关节置换术较锁定钢板内固定术具有手术操作简单、创伤较小、卧床时间短且并发症少的优点,值得推广应用。【Objective】 To analyze the differences in the curative effect of hemiarthroplasty and locking compression plate in the treatment of intertrochanteric fracture in old patients. 【Methods】 Fifty-two old patients with intertrochanteric fracture were collected. The patients were treated with hemiarthroplasty or locking compression plate at random. The operation time, bleeding volume during operation, incidence of complications after operation, the bedridden time after operation and hip joint function were compared within one year after operation between both groups. 【Results】 All the patients were followed up. The average operation time and the bedridden time after operation for hemiarthroplasty were shorter. The average bleeding volume and general complications for hemiarthroplasty were comparatively less. There were statistically significant differences in the four indexes between the hemiarthroplasty group and the locking compression plate group（P〈0.05）. The Harris scores at one year after operation were not statistically different between both groups（P〉0.05）. 【Conclusions】In the treatment of femoral intertrochanteric fracture in old patients, hemiarthroplasty is better than locking compression plates for easier operation, smaller wound, shorter bedridden time and fewer complications. Therefore, hemiarthroplasty is worth promoting the application.
