SLIPA喉罩与插管全麻在老年患者全膝关节置换术中的应用对比  

Comparison of Application About General Anesthesia with SLIPA Laryngeal Mask and Endotracheal Intubation for Total Knee Arthroplasty in Gerontal Patients

摘　　要：目的 比较喉罩和气管插管用于老年患者全膝关节置换术的安全性和临床效果。方法 68例患者随机分成SLIPA喉罩（S）和气管插管（E）两组,监测并记录两组麻醉诱导前（T0）、罩（管）置入成功即刻（T1）、罩（管）置入后3 min（T2）、罩（管）拔除即刻（T3）和罩（管）拔除后3 min（T4）的SBP、DBP、HR及Sp O2的变化,以及两组的不良反应。结果 S组置入时间明显小于E组时间（P〈0.001）,术毕拔管时间和苏醒时间小于E组（P〈0.05）;与T0相比,E组T1-T4时点的SBP、DBP明显升高,HR明显加快（P〈0.05）,S组无明显差异;E组呛咳和咽痛的发生率明显高于S组。结论 老年患者全膝关节置换术中采用SLIPA喉罩全麻,患者血流动力学更平稳,术后并发症较少,安全性高。Objective To compare the safety and clinical effect of general anesthesia with SLIPA laryngeal mask and endotra-cheal intubation for total knee arthroplasty in gerontal patient. Methods Sixty - eight patients were randomly assigned to SLIPA laryn-geal mask（S）group and endotracheal intubation（E）group,SBP,DBP,HR,SpO2 were recorded before induction of anesthesia （T0）,cover（tube）into successful immediate（T1）,3 min after（T2）,the cover（tube）removed instantly（T3）and cover（tube） after extraction of 3 min（T4）,and recorded complications of two groups. Results The times of insertion in S group were obviously lower than E group（P ﹤ 0. 001）;times to extubation and palinesthesia were lower than E group（P ﹤ 0. 05）;In group E,SBP,DBP and HR at T1 - T4 were higher than those at T0,but no significant difference in group S;Compared with group S,the incidences of bucking and sore throat of group E were much higher. Conclusion The application about general anesthesia with SLIPA laryngeal mask in total knee arthroplasty in gerontal patient make the hemodynamics steadily,less postoperative complications and highly safety.
