腰脊神经后支射频热凝术结合椎间关节注射技术在60例腰椎椎间关节综合征中的应用临床观察

摘　　要：[背景]评价数字血管减影机引导下腰脊神经后支射频热凝术结合椎间关节注射技术治疗腰椎椎间关节综合征的临床疗效及安全性.[病例报告]选择120例腰椎椎间关节综合征患者,随机分为椎间关节注射组(F组,n=60)和腰脊神经后支射频热凝术结合椎间关节注射组(FB组,n=60).比较治疗前后1周、1,3,6个月视觉模拟评分(VAS)、生存质量评分(QOL)及疼痛缓解率,并评估介入操作的安全性.结果见,治疗后FB组和F组的VAS评分较治疗前均明显降低(P<0.05),治疗后1周、1,3,6个月时FB组VAS,QOL及疼痛缓解率均明显优于F组(P<0.05).[讨论]腰脊神经后支射频热凝术结合椎间关节注射技术治疗腰椎椎间关节综合征效果满意,且安全可靠.BACKGROUND To evaluate the efficacy and safety of the digital subtraction angiography（DSA）guided-lumbar spinal nerve medial branch radiofrequercy thermocoagulation combined with zygapophysial injections in the treatment of lumbar facet joint syndrome.CASE REPORTS A total of 120 cases of lumbar facet joint syndrome were randomly divided into groups of the zygapophysial injection（group F,n=60）and lumbar spinal nerve medial branch radiofrequercy thermocoagulation combined with zygapophysial injection（group FB,n=60）.The visual analogue scale（VAS）,quality of life（QOL）,remission rate of pain and the safety of operation were observed before and after treatment for 1 week,1,3and 6months.The scores of VAS in both F and FB groups were significantly decreased after treatment than before treatment（P〈0.05）,and after treatment for 1week,1,3and 6 months,the scores of VAS,QOL and remission rate of pain were significantly decreased in FB group than in F group（P〈0.05）.DISCUSSIONThe lumbar spinal nerve medial branch radiofrequercy thermocoagulation combined with zygapophysial injection for treating lumbar facet joint syndrome is satisfied efficacy,safe and reliable.
