RKIP和VEGF在喉鳞癌及喉鳞状上皮不典型增生组织中的表达及临床意义

摘　　要：目的通过检测Raf-1激酶抑制蛋白(RKIP)、血管内皮生长因子(VEGF)在喉鳞癌及喉鳞状上皮不典型增生组织中的表达情况,分析二者在喉鳞癌发生、发展及转移中的作用及可能分子机制。方法采用免疫组化方法检测43例喉鳞癌组织、20例喉鳞状上皮不典型增生组织及9例正常喉黏膜组织中RKIP、VEGF的表达情况,分析其临床意义及二者之间的相关性。结果正常喉黏膜组织、喉鳞状上皮不典型增生组织及喉鳞癌组织中RKIP表达阳性率逐渐降低,VEGF表达阳性率逐渐上升,喉鳞癌组织中RKIP表达阳性率显著低于正常喉黏膜组织及喉鳞状上皮不典型增生组织(P均<0.05);有转移的喉鳞癌组织中RKIP表达阳性率低于无转移的喉鳞癌组织(P<0.05),VEGF表达阳性率高于无转移的喉鳞癌组织(P<0.05)。在正常喉黏膜组织、喉鳞状上皮不典型增生组织及喉鳞癌组织中,RKIP与VEGF的表达呈显著负相关(r=-0.324,P=0.006)。结论 RKIP具有抑制喉鳞癌发展及转移的作用,这可能是通过下调VEGF表达来实现的。Objective It is to observe the expressions and clinical significance of Raf - 1 kinase inhibitor protein （ RKIP）, vascular endothelial growth factor （VEGF） in laryngeal squamous cell carcinoma and laryngeal squamous epithelium atypical hyperplasia, and analyze the action of RKIP, VEGF in the occurrence and development of laryngeal squamous cell carcinoma as well as the possible molecular mechanisms. Methods Immunohistochemistry was used to detect the expressions of RKIP and VEGF in 43 cases of laryngeal squamous cell carcinoma tissue, 20 cases of laryngeal squamous epithelium atypical hyperplasia tissue and 9 cases of normal larynx squamous epithelium tissue. The clinical significance and correlation of them were ana- lyzed. Results The expression of RKIP in normal larynx squamous epithelium tissues, laryngeal squamous cell carcinoma tis- sues and laryngeal squamous cell carcinoma tissues was decreased gradually, and the positive rate of VEGF expression was in- creased gradually. RKIP expression in laryngeal squamous cell carcinoma tissues was lower than that in normal larynx squa- mous epithelium tissues and laryngeal squamous epithelium atypical hyperplasia tissue （ all P 〈 0.05 ）. RKIP expression in squamous cell carcinoma tissues with metastasis was significantly lower than that in squamous cell carcinoma tissues without metastasis （P 〈 0.05） , and VEGF expression was significantly higher than that in squamous cell carcinoma tissues without metastasis （P 〈0.05 ）. In all cases, RKIP expression had negative correlation with VEGF expression （ r = - 0.324, P = 0. 006）. Conclusion The RKIP may have the action of inhibition migration and metastasis in laryngeal squamous cell carcino- ma by down-regulating VEGF expression.
