中西医结合治疗下肢深静脉血栓形成的研究

摘　　要：目的观察中西医结合治疗下肢深静脉血栓形成的临床疗效。方法将108例下肢深静脉血栓形成患者随机分为治疗组和对照组,每组54例。对照组给予抗凝、溶栓、祛聚、活血化瘀、促进静脉淋巴回流消肿治疗,治疗组在对照组治疗基础上加用中药内服。观察2组治疗10 d后综合疗效,比较2组健患侧下肢周径差变化,检测治疗前及治疗10 d后D-二聚体水平,统计2组症状缓解时间、住院时间、并发症发生情况,随访1年内复发及下肢深静脉血栓形成后综合征发生情况。结果治疗组10 d治愈率明显高于对照组(P<0.05),健患侧下肢周径差明显小于对照组(P<0.05);2组治疗10 d后D-二聚体水平均较治疗前明显降低,但2组间比较差异无统计学意义(P>0.05);治疗组症状缓解时间及住院时间均明显短于对照组(P均<0.05);2组并发症发生率比较差异无统计学意义;随访1年,2组复发率比较差异无统计学意义(P>0.05),但治疗组下肢深静脉血栓形成后综合征发生率明显低于对照组(P<0.05)。结论中西医结合治疗下肢深静脉血栓形成可较快缓解症状,缩短住院时间,并可减少深静脉血栓形成后综合征的发生。Objective It is to observe the clinical effect of combined treatment of traditional Chinese medicine and western medicine in deep venous thrombosis （DVT） of lower limbs. Methods 108 patients with DVT of lower limbs were randomly di- vided into control group and treatment group, and 54 cases in each group. The patients were treated by antieoagulation, throm- bolysis, antiplatelet, activating blood and resolving stasis and promoting venous drainage to relieve swell of limbs in the control group, and plus oral traditional Chinese drugs on the basic of the above treatment in the treatment group. The comprehensive effects of two groups were observed after treatment for 10 clays; and the difference between the two groups in the lower limb circumference difference were compared; the levels of D - Dimer were detected before and after treatment; the symptom relief time, hospitalization time and complications occurred were observed in the two groups; the 1 - year recurrence rate and the oc- currence of post-thrombotic syndrome in lower extremity were followed up. Results In the treatment group, the recovery rate was markedly higher than that in the control group （P 〈 0.05） , the lower limb circumference difference was significantly less than that in the control group （ P 〈 0.05 ）. The levels of D - Dimer were markedly lower after treatment in both groups （ all P 〈 0. 05 ） , but there was no significant difference between the two groups（ P 〉 0. 05 ）. The symptom relief time and Hospitaliza- tion time in the treatment group were significantly shorter than those in the control group （ all P 〈 0. 05）. There was no signifi- cant difference in the complication rate between the two groups （ P 〉 0. 05 ）. Followed up for 1 - year, there was no significant difference in the recurrence rate between the two groups（ P 〉 0. 05 ）, but the incidence rate of post-thrombotic syndrome in the treatment group was markedly lower than that in the control group （P 〈 0.05 ）.
