扩张型心肌病患者微伏级T波电交替发生与心电指标关系探讨

摘　　要：目的研究扩张型心肌病患者T波电交替(T-wave alternans,TWA)的发生与QT间期、QT离散度、QRS时限等心电指标的关系。方法 31例扩张型心肌病患者,用Cambridge Heart.HeartwaveTM system心脏诊断系统,以频谱法检测MTWA(Microvolt T-wave alternans,微伏级T波电交替)。对所有患者进行体检、12导体表心电图检查。体表心电图测量QRS时限、QT间期、QT离散度等。分析MTWA的发生与QRS时限、QT间期、QT离散度等的关系。结果扩张型心肌病MTWA阳性组与MTWA阴性组的QT间期、QT离散度、QRS时限等心电指标无显著性差异。扩张型心肌病QRS≥120 ms组与QRS<120 ms组MTWA阳性率无显著性差异。结论扩张型心肌病患者MTWA阳性组与MTWA阴性组在QRS时限、QT间期、QT离散度等心电指标方面无差异。Objective To investigate relation of MTWA with the electrocardioindexes,such as QRS duration,QT interval,QT dispersion（QTd）and so on.Methods 31 patients with dilated cardiomyopathy including 30 men and 1 woman aging from40 to 61（50.06±7.14）years were selected.MTWA was assessed for all the patients with dilated cardiomyopathy with spectral method using Cambridge Heart.HeartwaveTM system.Physical examination and 12-lead surface electrocardiogram were conducted for all the patients with dilated cardiomyopathy.QRS duration,QT interval,QTd,et al were measured by way of 12-lead surface electrocardiogram.The relation of MTWA with QRS duration,QT interval,QT dispersion（QTd）et al was analyzed.Results There were no significant differences in QRS duration,QT interval,QTd,et al between positive MTWA and negative MTWA groups of patients with dilated cardiomyopathy（P〉0.05）.There were no significant differences in the MTWA positive rate between QRS≥120 ms and QRS〈120 ms groups of patients with dilated cardiomyopathy（P〉0.05）.Conclusion There are no significant differences in electrocardioindexes including QRS duration,QT interval,QTd and so on between positive MTWA and negative MTWA groups for patients with dilated cardiomyopathy.
