精索静脉曲张手术治疗对不育患者精子DNA片段化指数的影响

摘　　要：目的研究精索静脉曲张外科手术治疗对男性患者精液参数和精子DNA片段化指数的影响。方法回顾我院2012年1月-2013年6月接受精索静脉曲张手术的男性患者52例,收集患者术前1周和术后3个月的精液标本,研究精子DNA的完整性,测定DNA片段化指数(DFI),用流式细胞仪分析精子的染色质结构。结果相比术前,精索静脉曲张手术后3个月患者的精子计数和精子活力明显增加(P<0.05),精子DNA片段化指数由术前的42.5%下降为20.1%(P<0.001)。相比Ⅰ、Ⅱ度精索静脉曲张的患者而言,Ⅲ度精索静脉曲张患者术后的精子数呈现最大的改善。结论精索静脉曲张手术治疗可以提高多个精液参数,减少精子DNA的损伤,患者术前精液的参数缺陷在术后得到极大的改善。Objective The aim of this study was to evaluate the parameters of semen and the DNA fragmentation index （DFI） in patients with varicocele before and after varieoceleetomy. Methods The details of 52 consecutive patients were retrospectively analyzed from January 2012 to Jue 2013. The sperm samples were evaluated according to the World Health Organization Guidelines. Sperm DNA damage, characterized as DFI, was evaluated by sperm ehromatin structure assay using flow eytometry. Results Compared with the preoperative, three month after the operation, the number of sperm counts and sperm vitality was increased significantly （P〈0.05）, sperm DNA fragmentation index reduced from 42.5% to 20.1% after operation （P〈0.001）. Compared toⅠ , Ⅱ degree patients,Ⅲ degree patients with varioeele presented biggest improvement in sperm count after operation. Conclusion Varieoceleetomy can improve muhiple semen parameters and sperm DNA damage in infertile men with varieoeele. The patients with preoperative defects in those parameters showed greater improvement postoperatively.
