曲马多合并氟哌利多对人工流产中米索前列醇宫颈上药引起的寒战的治疗作用

摘　　要：目的观察曲马多合并氟哌利多对人工流产中米索前列醇宫颈上药引起的寒战的治疗效果。方法选取2013年2月至2014年12月期间要求无痛人工流产的早孕孕妇60例,随机分成A、B两组。在无痛人工流产术前2 h给予病人米索前醇宫颈上药后,A组(曲马多组)给曲马多注射液1 mg/kg加氟哌利多注射液2.5 mg静脉注射,B组(空白对照组)给予生理盐水3 ml静脉注射。观察记录A、B两组于术前120 min(T1),90 min(T2),60 min(T3),30 min(T4),术中(T5),术后60 min(T6)内寒战发生率及程度。同时观察记录A、B两组病人麻醉苏醒后下腹痛的发生率及程度。结果 A组病人寒战发生率及程度明显低于B组(Uc=2.120,P=0.034),A组病人麻醉苏醒后下腹痛的发生率及程度明显低于B组(Uc=5.940,P=0.000)。结论曲马多合并氟哌利多可有效减少人工流产中米索前列醇宫颈上药引起的寒战的发生,同时能缓解麻醉苏醒后子宫收缩引起下腹痛。Objectine To observe the efficacy of combined tramadol with droperidol in treatment of shivering induced by cervical adminis-tration of misoprostol in artificial abortion. Methods A total of 60 early pregnant women with painless artificial abortion induced by cervical ad-ministration of misoprostol during February 2013 to December 2014 were allocated in this study,they were randomly divided into A and . B two groups. In patients with painless abortion,the application of misoprostol at anterior cervix was given at 2 h before artificial abortion,patients in group A were given with tramadol,1 mg/ kg intravenous droplet infusion plus droperidol 2. 5 mg,and patients in group B（control group）were in-travenously administrated with 3 ml 0. 9% saline. The rates and degree of shivering were recorded in 120 minutes before the artificial abortion （T1）and 90 minutes（T2）,60 minutes（T3）,30 minutes（T4）,intraoperation（T5）and 60 minutes（T6）after operation. At the same time,the rate and extent of lower abdominal pain were observed and recorded in patients of these 2 groups after waking up from the anesthesia. Results The incidence and degree of shivering in patients of group A were significantly lower than those of patients in group B（Uc = 2. 120,P = 0. 034）. The rate and degree of abdominal pain in patients of group A after waking - up from anesthesia were significantly lower than those of patients in group B（Uc = 5. 940,P = 0. 000）. Conclusion Tramadol combined with droperidol can effectively reduce the occurrence of shivering during artificial a-bortion with cervical administration of misoprostol,at the same time it can remit the pain caused by contraction of uterine after anesthesia.
