中药外敷加口服治疗宫颈癌盆腔淋巴结清扫术后淋巴液渗出及预防淋巴囊肿形成的研究

摘　　要：目的研究中药外敷加口服治疗宫颈癌盆腔淋巴结清扫术后淋巴液渗出及预防淋巴囊肿形成的疗效。方法回顾分析宫颈癌行根治性子宫切除加盆腔淋巴结清扫术后第三天引流液(24 h)超过100 ml的患者75例,按治疗方法分为观察组和对照组。观察组采用中药外敷结合中药口服治疗,对照组采用单纯中药外敷治疗。对比观察两组治疗3 d后腹腔引流量和盆腔淋巴囊肿发生率。结果观察组治疗3 d后腹腔引流平均值(18.39±7.82 ml)较对照组(28.90±13.17 ml)明显减少;盆腔淋巴囊肿发生率低(5.56%)明显低于对照组(23.08%),差异均具有显著性(P<0.05)。结论中药外敷加口服治疗宫颈癌盆腔淋巴结清扫术后淋巴液渗出效果优于单纯中药外敷,并且可有效预防盆腔淋巴囊肿的形成。Objectine To study the external application and oral administration of traditional Chinese medicine in treatment of lymphatic fluid seepage and prevention of postoperative lymphocyst after lymph node dissection in patients with cervical cancer. Methods The clinical data of 75 patients with cervical cancer after radical hysterectomy and lymph node dissection had been retrospectively analyzed,the amount of postopera-tive drainage of fluid was more than 100 ml on the third day（24 hours）after operation. According to the therapeutic methods,all patients were di-vided into observation group and control group. Patients in observation group were treated with traditional Chinese medicine for external application and oral administration,and patients in control group were only treated with traditional Chinese medicine for external application. The amount of drainage from abdominal cavity and incidence of lymphocyst in 3 days after treatment had been compared between these 2 groups. Results In comparison with control group（28. 90 ± 13. 17 ml）,the average amount of drainage from abdominal cavity in patients of observation group（18. 39 ± 7. 82 ml）in 3 days after treatment had been declined. and the incidence of lymphocyst（5. 56% ）was significantly less than that of control group（23. 08% ）. The difference between these 2 groups was significant（ P ﹤ 0. 05）. Conclusion The clinical effect of lymphatic fluid seep-age and postoperative lymphocyst after dissection of lymph nodes in patients with cervical cancer after treatment with traditional Chinese medicine for external application and oral administration was better than those only with external application of traditional Chinese medicine,and it can effec-tively prevent the occurrence of lymphocyst.
