登革热实验室诊断指标准确率的研究

摘　　要：目的了解广州地区2014年登革热NS1抗原阳性与登革热病毒RNA阳性符合率及临床分布,并比较二者的阳性率差异以提高临床诊断准确率。方法收集2014年10月8日至31日广州市第一人民医院疑似登革热患者NS1抗原资料690份和病毒RNA资料315份,并留取其相应的标本检测血清NS1抗原和病毒RNA,统计二者的阳性符合率,利用χ2检验比较二者阳性率的差异。结果 690份检测登革热NS1抗原的患者中有437份NS1抗原阳性,阳性率为63.33%。315份检测登革热病毒RNA的患者有185份登革热病毒RNA阳性,阳性率为58.73%。79例登革热病毒RNA阳性结果中有74例NS1抗原结果阳性,二者阳性符合率为93.67%,其阳性率比较差异无统计学意义(P=0.069)。结论登革热RNA和登革热NS1抗原对于登革热患者的确诊均具有诊断价值,前者相对于后者准确性更高。Objective To understand the detection results and clinical distribution for two indicators about dengue virus NS1 antigen and dengue virus RNA in Guangzhou in 2014 and improve the clinical diagnostic accuracy. Methods Through accessing and inspecting the clinical records of patients, 690 results of dengue virus NS1 antigen test, and 315 results of dengue virus RNA in Guangzhou No.1 People＇s Hospital in October 2014 were collected. The positive rate and the positive coincidence rate of two indicators were calculated by using statistical software. Results 437 samples were positive（positive rate 63.33%） in 690 dengue virus NS1 antigen detection results. 185 samples were positive（positive rate 58.73%） in 315 dengue virus RNA detection results. 74 dengue virus NS1 antigen detection were positive in 79 dengue virus RNA detection positive results. Positive coincidence rate was 93.67%.There was no significant difference between the two groups（P =0.069）. Conclusion Dengue virus NS1 antigen and dengue virus RNA have diagnostic value for dengue fever diagnosis.
