摘　　要：目的了解和掌握深圳市龙岗区腹泻患者中致泻性大肠埃希菌感染状况及菌型分布,为临床和预防提供科学依据。方法采集2010-2013年在哨点监测医院（龙岗区人民医院）就诊的腹泻患者粪便标本3 638份,对致病菌进行分离、培养和鉴定,并对阳性菌株感染年龄段、流行季节及血清学进行分析。结果 3 638份腹泻标本共分离到490株致泻性大肠埃希菌,阳性率为13.47%,阳性菌株以肠产肠毒素性大肠埃希菌（ETEC）（231/490）和肠致病性大肠埃希菌（EPEC）（212/490）为主,各年间阳性检出率差异具有统计学意义（χ2=126.74,P〈0.05）;不同年份不同季节致泻性大肠埃希菌的感染情况差异有统计学意义;不同年龄组的感染情况差异有统计学意义,以10岁以下儿童（59/1430）和30~40青壮年（54/641）居多。结论龙岗区致泻性大肠埃希菌阳性病例以ETEC和EPEC为主;应重视辖区内腹泻病例的致泻性大肠埃希菌感染状况,提高暴发流行的预警能力。Objective To understand the situation of diarrheogenic Escherichia coil infection and distribution of various diarrheogenic Escherichia coil serotypes in diarrhea patients in Longgang District,Shenzhen. Methods 3638 stool samples were collected from diarrhea outpatients. Pathogenic bacteria were separated,cultivated and identified. The age distribution,incidence season and serotype were analyzed. Results The total positive rate of the pathogenic Escherichia coli was 13.47% in 3638 stool samples. The strains were mainly ETEC（231 / 490）and EPEC（212 / 490）. There were significant differences in the positive rates in different years（χ2=126.74,P〈0.05）, different seasons and different age groups. Infection mainly occurred among children under the age of 10（59 / 1430）and 30 ~40 young adults（54 / 641）.Conclusion Longgang District of diarrheagenic Escherichia coli positive cases were mainly caused by ETEC and EPEC strains. We should pay attention to the area of diarrhea cases caused by Escherichia coli infections, and raise the outbreak early warning capability.
