摘　　要：为促进整体护理模式下病人的全程管理,探索出院病人专业化延续护理的方案,从2012年6月至今,我院大力推进专科护理门诊的运行与发展,门诊年均接诊量逐年攀升,初步形成了出院病人全程管理的优质服务,有效降低了再入院率,符合护理人员的职业发展需求,推进了护理学科建设。本文对专科护理门诊的实践进行报道,阐述我院专科护理门诊目前的优势与不足,以期为推进专科护理门诊实现出院病人专业化延续护理提供参考。To promote the overall management of patients in the holistic nursing mode, and explore the plan for the discharged patients to continue specialized nursing, our hospital have promoted the operation and development of nursing specialist clinic vigorously from June 1, 2012 to May 31, 2015, annual outpatient visits increase steadily, and high quality service of overall management for the discharged patients has formed initially. In the holistic nursing mode, nursing specialist clinic promotes the overall patient management, effectively reduce the rate of re-admission, meet the demand of nurses＇ professional development and promote nursing discipline. This article discuss the practice of nursing specialist clinic, advantages and disadvantages of nursing specialist clinics in our hospital, in order to provide reference for promoting and realizing the continuation of professional nursing for discharged patients.
