“四查八对”在杜绝临床护理差错中的效果观察

摘　　要：目的：观察“四查八对制度在预防临床护理差错中的应用效果。方法在临床护理操作中执行“四查八对制度，即在原“三查七对的基础上增加一查一对，即增加患者或家属参与查；增加药物质量和有效期的核对。观察实施“四查八对制度前后护理差错发生率、纠错率。结果执行“四查八对制度后护理差错发生率由0.23％下降至0.13％，（χ2＝7.89），给药质量和有效期相关事件由10.3％下降至0，给药差错发生率由0.12％下降至0.05％，（χ2＝8.29）纠错率由0.01％提高至0.04％（χ2＝4.24），差异均有统计学意义（P＜0.05）。结论“四查八对作为护理核心制度应用于临床，保障了护理安全措施的进一步落实，较“三查七对具有优越性，且具有可行性，值得推广应用。Objective To investigate the effect of ＂ Four Inspections and Eight Verifications＂ on preventingclinical nursing errors. Methods On the basis of the analysis about the nursing errors from our department of nursing in 2009, based on the request of the ten safety goal,＂ Four Inspections and Eight Verifications＂ was set up and carried out.＂ Four Inspections and Eight Verifications＂ was executed into practice in clinical care operations, into which one Inspection and Verification was added on the basis of ＂Three Inspections and Seven Verifications＂ . That was, the patients or their families participating to the survey, drug quality and validity are added. The rate of error occurrence and error correction were observed. Results Compared to the control group of ＂Three Inspections and Seven Verifications＂, the nursing error rate dropped from 0.23% to 0. 13% （X2 =7.89, P〈0. 01 ）, the error rate of drug release decreased from 0. 05% to 0. 12% （X2 =8.29, P〈0. 01 ）, the error rate of correction increased from 0. 01% to 0. 04% （X2 =4.24, P〈0. 05 ）. There was statistical significance between these two groups, which could guarantee the further implementation of the nursing safety measures. Conclusions It has superiority and feasibility, and is worthy of wide application compared with Three Inspections and Seven Verifications.
