摘 要:糖尿病周围神经病变(Diabetic peripheral neuropa thy DPN)是糖尿病最常见的并发症之一,在我国DPN的发病率占糖尿病患者的60%以上,最多见的是远端对称性多发性神经病变和糖尿病自主神经病变。其病理改变主要为神经远端变性,神经纤维丧失,阶段性脱髓鞘和髓鞘再生。关于DPN的发病机制至今尚未完全明确,目前研究认为其主要与高血糖毒性、氧化应激、微血管病变及自身免疫等多因素相互作用,导致神经细胞损伤,神经纤维缺血性脱髓鞘有关。本文通过对近年来国内外DPN发病机制进行综述,为临床治疗提供参考。Diabetic Neuropathy CDiabetic peripheral neuropathy DPN) is one of the most common complication of diabetes, the incidence of DPN accounted for more than 60% of patients with diabetes, the most common is distal symmetric polyneuropathy, and diabetic autonomic neuropathy. Its main pathological, distal nerve degeneration, loss of nerve fibers, stage demyelination and remyelination. About pathogenesis of DPN, it has not yet fully clear. Present study suggests that the main hyperglycemia toxicity, oxidative stress, microvascular disease, and autoimmune and other factors interact, leading to nerve cell damage. This paper studies the pathogenesis of DPN and abroad in recent years were reviewed to provide reference for clinical treatment.
