台州地区1013例变应性鼻炎患者变应原谱分析

摘　　要：目的了解台州地区变应性鼻炎患者的变应原分布情况,为变应性鼻炎患者的预防及诊治提供参考。方法采用皮肤点刺试验对1 013例具有变应性鼻炎症状的患者进行变应原检测,分析检测结果,并探讨阳性病例在性别、年龄、检测季节等方面的差异性。结果 1 013例患者中,阳性反应者811例,阳性率为80.06%。其中,粉尘螨、屋尘螨过敏者为759例,占93.59%;葎草花粉过敏者248例,占30.58%;海蟹过敏者302例,占37.24%。吸入性变应原为主要致敏源,其阳性率为75.52%(765/1 013);食物性变应原阳性率为20.34%(206/1 013)。结论台州地区变应性鼻炎主要是由螨引起的变应性鼻炎,其次是由海蟹、葎草花粉、蟑螂等引起的变应性鼻炎,海虾、猫毛、狗毛、花生、扇贝、桃子等引起的变应性鼻炎较低,屋尘螨和粉尘螨阳性率呈正相关。明确变应原对变应性鼻炎的防治具有重要的指导意义。Objective To determine the distribution of allergens among allergic rhinitis patients living in Taizhou city and provide reference on taking precautions and treatment for allergic rhinitis patients. Methods 1 013 patients with allergic rhinitis were recruited for allergens by skin prick test. By analyzing testing results to explore difference among positive tests patients in sex,age and testing season. Results There were 811 cases with positive prick test in the 1 013 patients,the positive rate was80. 06%. Among the 811 patients with positive prick test,the allergens were dermatophagoides culinae and house dust mite in759 cases accounting for 93. 59%,pollen in 248 cases accounting for 30. 58%,sea crab in 302 cases accounting for 37. 24%.Inhalation allergen is the major sensibiligen accounting for 75. 52%（ 765 /1 013） and food allergen accounts for 20. 34%（ 206 /1 013）. Conclusion The most common allergen in patients with allergic rhinitis in Taizhou city is dermatophagoides culinae and house dust mite. The second is sea crab,pollen and cockroach,etc. The prawn,cat fur,dog hair,peanut,pectinidae and peach are rare as allergens among the allergic rhinitis patients in Taizhou city. The positive rate of dermatophagoides culinae and house dust mite was positively correlated. Confriming the specific allergen may provide the guideline to treatment allergic rhinitis patients.
