急性心肌梗死患者经皮冠状动脉介入治疗术后脑钠肽、血清缺血修饰蛋白、超敏C反应蛋白的表达

摘　　要：目的探讨急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术前、术后脑钠肽(BNP)水平、缺血修饰蛋白(IMA)、超敏C反应蛋白(hs-CRP)表达的临床意义。方法选择65例接受PCI治疗的AMI患者术前、术后1 h、3 h、6 h、24 h、48 h、1周,抽取静脉血检测BNP、IMA、hs-CRP,对检测结果进行分析。结果 BNP水平在术后逐渐下降,术后3 h与术前比较差异有统计学意义(P<0.01),术后48 h明显降低(P<0.001);IMA在术后逐渐升高,3 h时达到高峰,与术前比较差异有统计学意义(P<0.001),在术后24 h逐渐降至接近术前水平(P>0.05);hs-CRP在术后逐渐升高,峰值出现在术后24 h,与术前比较差异有统计学意义(P<0.001),在术后1周与术前比较差异无统计学意义(P>0.05)。结论 BNP、IMA、hs-CRP在PCI术前、术后的动态表达对疗效的观察、判断及病情预后具有重要的参考价值。Objective To explore the value of the expression of brain natriuretic peptide（ BNP）,ischemia modified albumin（ IMA） and hypersensitive C- reactive protein（ hs- CRP） levels in patients with acute myocardial infarction（ AMI） before and after percutaneous coronary artery intervention（ PCI） treatment. Methods Determine the levels of BNP,IMA and hs- CRP in65 patients with AMI before operation,at 1 h,3 h,6 h,24 h,48 h and one week after operation. Results The BNP levels decreased gradually after operation. There was statistical significance on the differences between preoperative level and the level at 3 h after operation（ P〈0. 01）. In addition,the level decreased significantly at 48 h after operation（ P 0. 001）. The IMA levels increased gradually after operation. It＇s level at 3 h after operation reached the peak,furthermore,which had statistical significance on the differences comparing with the preoperative level（ P 0. 001）. Henceforth,the level decreased to near the preoperative level at 24 h after operation（ P〈0. 05）. The hs- CRP levels also increased gradually after operation,and reached to the peak at 24 h after operation. Comparing with preoperative level,the peak had statistical significance on the difference（ P 〈0. 001）,but the level after one week had no statistical significance on the difference（ P〈0. 05）. Conclusion The levels of BNP,IMA and hs- CRP before and after PCI treatment are of important value to judge the efficacy of PCI and prognostic.
