摘  要:目的对南充地区耐碳青霉烯类鲍氏不动杆菌(CRAB)的碳青霉烯酶耐药基因进行检测分析,初步了解该地区CRAB耐药机制。方法收集南充市两所三甲医院临床感染患者送检标本检出的90株非重复CRAB,采用改良Hodge试验筛查碳青霉烯酶,PCR扩增技术检测OXA-23 like、OXA-24 like、OXA-51 like、OXA-58 like、IMP、VIM型碳青霉烯酶基因以及NDM-1"超级细菌"耐药基因。结果改良Hodge试验阳性共72株,阳性率80.0%;全部菌株携带OXA-51-like基因,79株携带OXA-23-like基因,检出率为87.8%,其余基因扩增均为阴性。结论该地区CRAB的耐药机制与产OXA-23型碳青霉烯酶有关,未发现其他碳青霉烯酶耐药基因参与。OBJECTIVE To detect the carbapenem resistance genes in the carvbapenem-resistant Acinetobacter baumannii(CRAB)isolated in Nanchong area and achieve preliminary understanding of the drug resistance mechanisms of the CRAB strains isolated in this area.METHODS A total of 90 strains of no-repetitive CRAB were collected from the patients with clinical infections in two three-A hospitals of Nanchong.The modified Hodge test was carried out to screen out carbapenemases.The OXA-23 like,OXA-24 like,OXA-51 Like,OXA-58 like,IMP,and VIM types of carbapenemases genes and NDM-1′superbugs′resistant gene were detected by using PCT amplification technique.RESULTS The modified Hodge test showed that totally 72 strains were detected positive with the positive rate of 80.0%,all of which carried OXA-51-like gene,79 strains carried OXA-23-like gene with the detection rate of 87.8%,and the rest of the genes were amplified negative.CONCLUSION The drug resistance mechanism of the CRAB strains isolated in the area is associated with the production of OXA-23 carbapenemase,and other carbapenem resistance genes are not detected to involve in the drug resistance mechanism.
