头孢哌酮/舒巴坦联合奥硝唑治疗妇科感染的疗效研究

摘　　要：目的探讨头孢哌酮/舒巴坦联合奥硝唑治疗妇科感染的临床疗效,为临床抗菌治疗策略的制定提供参考。方法选取医院收治的各类妇科感染患者292例,随机分为观察组和对照组,各146例;对照组采用头孢哌酮/舒巴坦静脉滴注进行治疗,观察组在对照组治疗方案的基础上,加用奥硝唑静脉滴注进行治疗;在治疗前后均采集相应部位的分泌物检测病原菌,对比两组患者的病原菌清除率、临床疗效和不良反应发生率。结果妇科感染病原菌检出率最高为大肠埃希菌,其次为肺炎克雷伯菌;两组患者治疗前均检出病原菌113株,检出率均为77.40%;观察组治疗后的病原菌清除率为90.27%,显著高于对照组(P<0.05);经过治疗后,观察组的总治愈率为51.37%,总有效率为81.51%,对照组分别为36.99%和71.23%,观察组的总治愈率和总有效率均显著高于对照组(P<0.05);在治疗过程中,对照组的不良反应发生率为17.12%,观察组的不良反应发生率为15.07%,差异无统计学意义。结论头孢哌酮/舒巴坦联合奥硝唑治疗可提高妇科感染的临床疗效,更有效地清除病原菌,且安全性高,值得临床推广应用。OBJECTIVE To investigate the clinical efficacy of cefoperazone-sulbactam combined with ornidazole in treatment of gynecologic infections so as to provide guidance for clinical antimicrobial treatment.METHODS A total of 292 patients with gynecologic infections who were treated in the hospitals were enrolled in the study and randomly divided into the observation group and the control group,with 146 cases in each.The control group was treated with intravenous drip of cefoperazone-sulbactam,while the observation group was given additional intravenous drip of ornidazole on the basis of the control group.The secretions specimens were collected from the corresponding sites before and after the treatment for the detection of pathogens.The clearance rates of pathogens,clinical curative effects,and incidence of adverse reactions were observed and compared between the two groups of patients.RESULTS The isolation rate of the Escherichia coli was the highest among the pathogens causing the gynecologic infections,followed by the isolation rate of the Klesiella pneumoniae.Totally 113 strains of pathogens were isolated from both groups before the treatment,with the isolation rates of 77.40%.The clearance rate of pathogens of the observation group was 90.27% after the treatment,significantly higher than that of the control group（P〈0.05）.After the treatment,the total cure rate of the observation group was 51.37%,significantly higher than 36.99% of the control group（P〈0.05）;the total effective rate of the observation group was81.51%,significantly higher than 71.23% of the control group（P〈0.05）.The incidence of adverse reactions was 17.12%in the control group during the treatment,15.07%in the observation group,and the difference was not significant.CONCLUSIONCefoperazone-sulbactam combined with ornidazole can improve the clinical curative effect on treatment of the gynecologic infections and eradicate the pathogens more effectively,with the safety high,and it is worthy to be promoted in the hospital.
