改进医院感染管理措施的成效分析

摘　　要：目的验证改进医院感染管理方法对提高医院感染控制管理效能的影响,探讨感染控制管理的新思路和新方法,以期促进医院感染管理更科学、规范、有序。方法回顾性调查2011年6月-2012年5月管理措施改进前出院患者3 248例和2012年6月-2013年5月改进后出院患者3 293例的医院感染率、漏报率、抗菌药物使用率、手卫生依从率等医院感染监测重点指标变化;采用SPSS15.0软件进行统计分析。结果改进前,医院感染率为3.96%、医院感染漏报率为57.52%、抗菌药物使用率为64.32%、手卫生依从率为34.26%;改进后医院感染率为2.64%、医院感染漏报率为11.73%、抗菌药物使用率为57.15%、手卫生依从率为85.69%,差异有统计学意义(P<0.05)。结论改进感染控制管理方法使医院感染率、医院感染漏报率、抗菌药物使用率均显著下降,手卫生依从率大幅提高;改进管理措施可有效提高医院感染管理质量、提高控制感染工作效能。OBJECTIVE To verify the effect of the improvement of nosocomial infection management measures on the control of nosocomial infections and explore the new ideas and ways of the nosocomial infection management so as to facilitate the scientific,standard,and orderly management of the nosocomial infections.METHODS A total of3 248 patients who were discharged from Jun 2011 to May 2012（before the improvement of the management measures）and 3 293 patients who were discharged from Jun 2012 to May 2013（after the improvement of the management measures）were enrolled in the study.The changes of the key monitoring indicators such as the incidence of nosocomial infections,missing report rate,utilization rate of antibiotics,and hand hygiene compliance were observed,and the statistical analysis was performed with the use of SPSS15.0software.RESULTS The incidence of nosocomial infections was 3.96% before the improvement,2.64% after the improvement;the missing report rate of the nosocomial infection cases was 57.52% before the improvement,11.73% after the improvement;the utilization rate of antibiotics was 64.32% before the improvement,57.15% after the improvement;the hand hygiene compliance was 34.26% before the improvement,85.69% after the improvement,and the difference was significant（P〈0.05）.CONCLUSIONThe improvement of the infection control measures may contribute to the remarkable reduction of the incidence of nosocomial infections,missing report rate of nosocomial infection cases,and utilization rate of antibiotics as well as the significant improvement of the hand hygiene compliance.It can effectively improve the quality of management of the nosocomial infections and raise the effectiveness of the control of the nosocomial infections.
