外剥内扎术配合加压缝合皮桥治疗环状混合痔79例

摘　　要：为观察外剥内扎术配合加压缝合皮桥治疗环状混合痔的疗效，将132例环状混合痔患者分为治疗组79例和对照组53例，治疗组行混合痔外剥内扎加压缝合法处理环状混合痔皮桥上的赘皮组织，其中有17．7％的患者术后皮桥上残留皮赘。一次手术切除，治愈率为100％，无肛管狭窄或肛门失禁等后遗症。对照组行混合痔外剥内扎术后，如皮肤残留的赘皮组织较多，采用横行切除、对合切口全层缝合法处理，其中有45．3％的患者术后皮桥上残留皮赘，一次手术切除，治愈率为100％，术后出现轻度肛管狭窄4例。结果表明，外剥内扎术配合加压缝合皮桥治疗环状混合痔疗效确切。This study was to observe the effect of external dissection ＆ internal ligation associated with pressure suturing skin bridge in the treatment of circumferential mixed hemorrhoids.The 132 cases of this hemorrhoids were divided into treatment group（79 cases） and control one（53 cases）;the former reeived a- bove combination procedure,i.e external dissection ＆ internal ligation for mixed hemorrhoids, while pres- sure suture for postoperative excrescent tissue on the skin bridge, 17.7% of the patients suffered from re- siduary skin tag,which had to be resected by surgery,thus curative rate was up to 100%,there was no se- quel such as stenosis or incontinence of anus,etc;in the control group after external dissection ＆ internal ligation for mixed hemorrhoids if it was of excrescent tissue in massive volume just adopting transversal re- section to remove the residuary skin tag, then managing it through full-thickness countersuture, but 45.3 % of the patients suffered from residuary skin tag which had to be resected by surgery,up to 100% of curative rate;after operation 4 cases had light anal stenosis. Results show that for circumferential mixed hemor- rhoids external dissection ＆ internal ligation associated with pressure suturing skin bridge has determined effect.
