外切整形联合PPH治疗重度环状混合痔疗效分析

摘　　要：为探讨外切整形联合PPH治疗重度环状混合痔的疗效，回顾性分析行外切整形联合PPH治疗的重度环状混合痔患者40例临床资料。结果显示，术后患者平均住院时间4．5d，创口愈合平均时间13．5d，术后皮缘水肿发生率5．0％（2／40），尿潴留发生率2．5％（1／40）。术后3d疼痛评分：第1天6．1分，第2天3．2分，第3天1．8分。随访12个月，发现便血2例，痔脱出1例，无肛门失禁及控便不良、肛门狭窄、肛周感染等并发症。结果表明，外切整形联合PPH治疗重度环状混合痔是一种安全可行的手术方法，既能彻底清除病灶，又整形修复了肛门外形，减少了并发症，获得较高的满意度。In order to explore the effect of external resection gc plasty combined with PPH in the treatment of severe circuferential mixed hemorrhoids,authors retrospectively analyzed the clinical data of 40 patients of this hemorrhoids who had received above combined procedure.As results,after operation average hospi- tal stay was 4.5 days, average wound healing time was 13.5 days, the incidence of skin-edge edema was 5.0% （2/40）, the incidence of uroschesis was 2.5 % （1/40） ; on the 1 st, 2nd and 3rd day after operation the pain score rating was 6.1,3.2 and 1.8, respectively; the followed-up of 12 months found that 2 cases suf- fered from hematochezia, 1 from hemorrhoid prolapse, but no incontinence and bad controlling stool, steno- sis,infection of anns,etc.Results show that above combined procedure is an available one to severe circum- ferential mixed hemorrhoids, not only throughly can remove lesion, but repair anal appearance plastically, reduce complication,so obtain higher satisfactory degree of patients.
