多层螺旋CT及血管成像对肝泡状棘球蚴血管受侵的诊断价值

摘　　要：目的探讨多层螺旋CT评价肝泡状棘球蚴侵犯血管的诊断价值及其侵犯血管的影像特征。方法25例肝脏泡状棘球蚴患者,术前均行多层螺旋CT三期增强扫描,进行三维血管重建,与术后结果进行对照,评价CT检查对肝动脉、门静脉、肝静脉及下腔静脉受侵的诊断效能。结果 25例中,肝动脉受累共有23支,门静脉受累39支,肝静脉受累37支,下腔静脉受累14支;与手术结果比较,多层螺旋CT(MSCT)对肝动脉、门静脉、肝静脉受累的诊断符合率分别为82.61%、92.31%、94.59%,进行一致性检验,kappa值分别为0.780、0.884、0.896,MSCT判断下腔静脉侵犯与手术结果完全相符合。结论多层螺旋CT能够较准确判断血管受侵的严重程度,能为术前客观评估提供可靠的影像学信息。Objective To evaluate the effectiveness of multi-slice spiral computerized tomography （MSCT） in patient with vascular invasion of the hepatic alveolar echinococcosis and to summary the image features. Methods 25 patients with hepatic alveolar echinococcosis were enrolled in this study. All the patients un derwent MSCT scan and three-dimensional reconstruction were performed before surgery. Then a comparison between the operation and the CT image were made for evaluation the hepatic vessels. Results In the 25 lesions, 23 branches of hepatic artery, 39 of portal vein, 37 of hepatic vein and 14 of inferior vena cava were involved. Compared with surgery, the Diagnostic coincidence rate of MSCT were 82.61% （artery）, 92.31% （portal vein）, 94.59% （hepatic vein）respectively and the kappa values were 0.780, 0.884, 0.896. The result of MSCT was completely coincident with surgery in judgment inferior vena cava involvement. Conclusion MSCT can accurately evaluated the invasion vessels. It can provide reliable imaging in formation for preoperative evaluation.
